Artemisinin combination therapy (ACT) is the first-line treatment for uncomplicated malaria recommended by the World Health Organization (WHO), particularly for infections caused by Plasmodium falciparum. ACT pairs a rapidly acting artemisinin-derived compound with a second antimalarial drug that has a longer half-life. This combination approach maximizes treatment effectiveness and significantly reduces the risk of the malaria parasite developing resistance to either drug.
Artemisinin and its derivatives – including artemether, artesunate, and dihydroartemisinin – act rapidly against the blood-stage malaria parasite. They generate reactive oxygen species (free radicals) that are toxic to the parasite, causing widespread cellular damage. Because artemisinin derivatives are eliminated from the body quickly, a slower-acting partner drug is always combined to clear any remaining parasites and prevent recrudescence (return of the infection).
ACT is typically administered orally over a three-day course. Dosing is weight-based, which is especially important when treating children. For severe malaria, intravenous or intramuscular artesunate is the preferred treatment. It is essential to complete the full course of treatment as prescribed in order to prevent the development of drug resistance.

A growing concern is the emergence of artemisinin resistance, first detected in Southeast Asia, particularly in the Greater Mekong Subregion. Resistant parasites show a slowed response to treatment. The WHO closely monitors resistance patterns worldwide and strongly advises against the use of artemisinin monotherapy, which is a major driver of resistance development.
The widespread adoption of ACT has dramatically reduced malaria mortality worldwide. In sub-Saharan Africa, where malaria remains endemic, ACT has saved millions of lives, especially among young children. It remains a cornerstone of the global malaria control and elimination strategy, supported by organizations such as the WHO, UNICEF, and the Global Fund.
