What is Autochthonous Malaria?

Autochthonous malaria is a malaria infection that a person acquires within their own country, without having traveled to a tropical or subtropical endemic region. The term “autochthonous” derives from Greek, meaning “native” or “originating in the same place.” Unlike imported malaria – where travelers bring the parasite back from abroad – autochthonous malaria is transmitted locally. This is considered an exceptional event in countries where malaria has been officially eliminated, such as in Central and Western Europe.

Causes and Transmission

Malaria is caused by parasites of the genus Plasmodium and is transmitted through the bite of an infected female Anopheles mosquito. In autochthonous malaria, this transmission occurs locally without the affected person having visited an endemic area. Possible scenarios include:

  • Local vectors: In some temperate countries, Anopheles mosquitoes still occur naturally. If such a mosquito has previously bitten an infected person, it can transmit the parasite locally.
  • Climate change: Rising temperatures may facilitate the spread of Anopheles mosquitoes into previously unaffected regions.
  • Airport malaria: A specific form of autochthonous malaria in which infected mosquitoes are transported via aircraft from endemic areas and infect people living near airports.
  • Cryptic malaria: Cases with no identifiable source of transmission, where neither travel history nor vector contact can be established.

Common Pathogens

The most common malaria parasites involved in autochthonous malaria in temperate zones include:

  • Plasmodium vivax: Capable of surviving in temperate climates; the most common species involved in autochthonous cases in Europe.
  • Plasmodium falciparum: More common in tropical regions but can be locally transmitted via imported mosquitoes.

Symptoms

The symptoms of autochthonous malaria are identical to those of imported malaria. Typical complaints include:

  • Fever episodes: Recurring, often cyclical fever (every 48 or 72 hours)
  • Heavy sweating and chills
  • Headache and muscle pain
  • General malaise, fatigue, and weakness
  • Nausea, vomiting, and diarrhea
  • Enlarged spleen and pallor (in prolonged cases)

Because malaria is rarely diagnosed in non-endemic countries, autochthonous cases carry a higher risk of delayed diagnosis, as clinicians may not consider this possibility without a travel history.

Diagnosis

Diagnosis is established through:

  • Blood smear and thick film: Microscopic examination of blood for Plasmodium parasites – the gold standard of malaria diagnostics.
  • Rapid diagnostic tests (RDT): Antigen-based tests for a quick initial diagnosis.
  • PCR (Polymerase Chain Reaction): A highly sensitive molecular method for species identification.

When autochthonous malaria is suspected, a thorough patient history is essential, including place of residence, possible mosquito exposure, and contact with returning travelers.

Treatment

Treatment depends on the Plasmodium species and the severity of the illness:

  • Plasmodium falciparum: Artemisinin-based combination therapies (ACT) are the standard of care.
  • Plasmodium vivax: Chloroquine (where still effective) combined with primaquine to prevent relapse (eradication of hypnozoites).
  • Severe malaria: Intravenous artesunate is the preferred treatment.

Early treatment is critical to prevent severe complications such as cerebral malaria, organ failure, or death.

Epidemiology and Relevance in Europe

Although malaria is considered eliminated in Europe, isolated autochthonous cases have been reported in recent years – particularly in southern Europe (e.g., Greece, Spain, Italy) and near airports. Climate change and increasing global travel raise the risk that local Anopheles mosquitoes become infected and establish local transmission chains. Public health authorities must carefully investigate and report such cases.

Prevention

  • Protection against mosquito bites (repellents, bed nets, protective clothing)
  • Surveillance and monitoring of Anopheles mosquito populations in at-risk areas
  • Rapid reporting and investigation of suspected cases to public health authorities
  • Education of healthcare professionals about the possibility of autochthonous malaria even without a travel history
