What is Baclofen?
Baclofen is a muscle relaxant primarily used to treat spasticity — a condition in which muscles remain abnormally tense or contract involuntarily. It belongs to the class of GABA-B receptor agonists and acts directly on the central nervous system (brain and spinal cord). Baclofen is available both as an oral tablet and as an intrathecal solution administered via an implanted pump.
Indications (Uses)
Baclofen is prescribed for a range of conditions associated with spasticity or muscle spasms:
- Multiple Sclerosis (MS): Relief of spasticity and painful muscle spasms
- Spinal cord injuries: Management of injury-related spasticity
- Cerebral palsy: Improvement of mobility in children and adults
- Stroke: Adjunct treatment of post-stroke spasticity
- Amyotrophic Lateral Sclerosis (ALS): Reduction of muscle stiffness
- Alcohol dependence: In some countries, baclofen is used off-label to support alcohol withdrawal therapy
Mechanism of Action
Baclofen acts as an agonist at GABA-B receptors in the brain and spinal cord. GABA (gamma-aminobutyric acid) is the main inhibitory neurotransmitter of the nervous system. By activating GABA-B receptors, baclofen inhibits the transmission of nerve signals that trigger muscle contractions. This results in relaxation of overactive muscles and a reduction in involuntary spasms.
Dosage and Administration
Baclofen is available in two primary forms:
- Oral tablets: The standard form for most patients. Dosage is individually adjusted and gradually increased to minimize side effects. Typical daily doses range from 15 to 80 mg, divided into three doses per day.
- Intrathecal pump: For severe, treatment-resistant spasticity, baclofen can be delivered directly into the spinal fluid via a surgically implanted pump. This method allows much lower total doses while achieving greater efficacy.
Important: Baclofen must never be stopped abruptly, as this can lead to dangerous withdrawal symptoms.
Side Effects
Like all medications, baclofen may cause side effects. Common side effects include:
- Drowsiness and sedation
- Dizziness and balance disturbances
- Nausea
- Muscle weakness
- Confusion, particularly in elderly patients
- Headache
Rare but serious side effects may include:
- Respiratory depression (slowed breathing)
- Liver function abnormalities
- Hallucinations
- Baclofen withdrawal syndrome upon abrupt discontinuation: This can manifest as fever, confusion, severe muscle rigidity, and in rare cases, life-threatening conditions.
Drug Interactions
Baclofen can interact with other medications. Special caution is advised with:
- Alcohol and other CNS depressants: Enhanced sedative effect
- Antihypertensives (blood pressure-lowering agents): Potentiation of blood pressure-lowering effects
- Antidepressants and antipsychotics: Increased risk of central nervous system side effects
- Lithium: Increased risk of hypertonia
Contraindications
Baclofen should not be used, or only used under strict medical supervision, in patients with:
- Known hypersensitivity to baclofen
- Severe renal insufficiency (kidney failure)
- Epilepsy (increased risk of seizures)
- Pregnancy and breastfeeding (only after careful benefit-risk assessment)
- A history of psychiatric disorders
