What is Benzocaine?
Benzocaine (also known as Benzocainum) is a local anesthetic belonging to the ester anesthetic group. It is widely used in over-the-counter products to temporarily relieve pain, itching, and burning sensations on the skin and mucous membranes. Benzocaine is a white, odorless powder that is poorly soluble in water but dissolves well in organic solvents.

Benzocaine is used in numerous medical and pharmaceutical applications, including:
- Toothaches and gum discomfort (e.g., teething in infants, canker sores)
- Sore throats (in lozenges and throat sprays)
- Sunburn and minor skin irritations
- Insect bites and itching

Mechanism of Action
Benzocaine belongs to the class of sodium channel blockers. It reversibly inhibits voltage-gated sodium channels in nerve cell membranes, thereby blocking the transmission of pain signals along nerve fibers without permanently damaging surrounding tissue. Unlike many other local anesthetics, benzocaine is barely absorbed into the bloodstream, as it acts almost exclusively on the surface (topically).
Dosage and Administration
Benzocaine is available in various dosage forms:
- Gels and ointments: Apply a thin layer to the affected area (typical concentration: 5–20 %)
- Lozenges and throat sprays: Use as directed on the package, generally no more than every 2 hours
- Ear drops: Use only under medical supervision
The use of benzocaine in children under 2 years of age should be avoided without medical advice, due to an increased risk of a rare but serious adverse effect (methemoglobinemia).
Side Effects
Benzocaine is generally well tolerated, but may in rare cases cause the following side effects:
- Allergic reactions: Skin rash, swelling, contact dermatitis
- Methemoglobinemia: A rare but serious condition in which the blood's ability to carry oxygen is impaired. Symptoms include bluish discoloration of the lips and skin (cyanosis), dizziness, and shortness of breath. Infants, young children, and individuals with certain enzyme deficiencies are at particular risk.
- Burning or tingling at the site of application
Interactions
Benzocaine may interact with certain other substances:
- Combination with other agents that can cause methemoglobinemia (e.g., nitrates, sulfonamides) increases the risk of this side effect.
- Medical advice should be sought when using benzocaine alongside blood thinners or other anesthetics.
Safety Considerations
The U.S. Food and Drug Administration (FDA) has issued warnings regarding the use of benzocaine products in children under 2 years of age. It is recommended to use benzocaine-containing products strictly according to the manufacturer's instructions or as advised by a healthcare professional. Persistent or severe symptoms should always be evaluated by a physician.
