What is Betahistine?

Betahistine is an antivertigo medication specifically used to treat dizziness and balance disorders. It belongs to the group of histamine analogues and acts on histamine receptors in the inner ear and the brain. It is primarily prescribed for Meniere's disease and other forms of vestibular vertigo. Common brand names include Serc®, and various generic formulations are widely available.

Indications

Betahistine is mainly used for the following conditions:

Meniere's disease: An inner ear disorder characterized by recurring episodes of severe dizziness (vertigo), tinnitus (ringing in the ears), and hearing loss.
Vestibular vertigo: Dizziness originating from the balance organ in the inner ear.
Tinnitus associated with inner ear disorders.

Mechanism of Action

Betahistine works through two complementary mechanisms on the histamine system:

H1-receptor agonism: Betahistine weakly activates H1 histamine receptors in the blood vessels of the inner ear. This leads to improved blood circulation in the labyrinth (inner ear) and helps to reduce elevated endolymphatic pressure, which is a key factor in Meniere's disease.
H3-receptor antagonism: Betahistine also blocks H3 histamine receptors in both the central and peripheral nervous system. This promotes increased histamine release and improves the processing of balance-related signals.

Through these combined effects, betahistine helps normalize vestibular signal transmission and reduces the frequency and severity of vertigo attacks.

Dosage

The standard dosage for adults is:

8 mg to 16 mg, two to three times daily, as directed by a physician.
Maximum daily dose: typically 48 mg.
Betahistine should be taken with or after meals to improve tolerability.
Treatment should be maintained over several weeks to months, as the therapeutic effect often develops gradually.

Side Effects

Betahistine is generally well tolerated. Possible side effects include:

Common: Nausea, gastrointestinal discomfort, headache.
Occasional: Skin reactions such as itching or rash.
Rare: Allergic reactions, weight changes.

Patients with a history of peptic ulcers, bronchial asthma, or phaeochromocytoma (a tumor of the adrenal gland) should only use betahistine under close medical supervision.

Drug Interactions

Betahistine may interact with the following substances:

Antihistamines (H1 blockers): May reduce the effectiveness of betahistine.
MAO inhibitors: Concurrent use may increase betahistine plasma levels.

Contraindications

Known hypersensitivity to betahistine or any excipient.
Phaeochromocytoma (due to histamine-like activity).
Children and adolescents (insufficient clinical data available).
