Bile acid therapy refers to the medical use of natural or synthetically produced bile acids to treat various diseases of the liver, bile ducts, and gallbladder. Bile acids are endogenous substances produced in the liver from cholesterol. They play a central role in fat digestion and the excretion of cholesterol. The most well-known therapeutically used bile acids are ursodeoxycholic acid (UDCA) and obeticholic acid (OCA).

The dosage of bile acid therapy depends on the underlying condition, body weight, and individual patient response. For UDCA in primary biliary cholangitis, the standard recommendation is 13–15 mg per kilogram of body weight per day, divided into multiple doses. In gallstone dissolution therapy, treatment may last from several months up to two years. Therapy is prescribed and monitored exclusively by a physician, with regular liver function testing.

Obeticholic acid can cause intensified itching (pruritus) at higher doses, making careful dose adjustment necessary. Both agents are generally not suitable for patients with decompensated liver cirrhosis.
Before initiating bile acid therapy, laboratory tests including liver enzymes (ALT, AST, GGT, alkaline phosphatase), bilirubin levels, and imaging procedures such as ultrasound are typically performed. Regular follow-up examinations during therapy are essential to assess treatment response and detect potential side effects early.
