Borderline personality disorder (BPD) is a severe mental illness characterised by an unstable self-image, extreme mood swings, impulsive behaviour and disturbed interpersonal relationships.

Affected people often experience emotions more intensely and impulsively than others and have difficulty regulating their feelings, which leads to recurring crises and severe inner suffering. Also typical are fear of abandonment, a chronic inner emptiness and a fluctuating image of themselves and others.

The causes of BPD are multifactorial: In addition to a genetic predisposition, traumatic childhood experiences, emotional neglect or unstable attachments often play a major role. Neurobiological changes, for example in the amygdala or the prefrontal cortex, which are responsible for emotional regulation, can also be partly responsible. The first signs often appear in late childhood or adolescence and can worsen in adulthood if no adequate therapy is provided.

The most effective form of therapy for BPD is dialectical behavioural therapy (DBT), a special form of behavioural therapy that helps those affected to control impulses, regulate emotions and build social skills. In addition, chemotherapy, mindfulness-based methods, psychoeducation and in certain cases medications for symptom treatment (e.g. antidepressants, mood stabilisers) can be used. Early diagnosis and specialised, long-term care are crucial to significantly improve quality of life.
