What is Bromhexine?

Bromhexine is a synthetic mucolytic drug used to loosen and thin thick mucus in the airways, making it easier to cough up. It belongs to the class of expectorants and mucolytics and is available in various forms, including tablets, syrups, drops, and inhalation solutions. It is used in both adults and children.

Indications

Bromhexine is indicated for use in:

  • Acute and chronic bronchitis
  • Chronic obstructive pulmonary disease (COPD)
  • Bronchial asthma with excessive mucus production
  • Bronchiectasis (abnormal widening of the bronchial tubes)
  • Cystic fibrosis as a supportive treatment
  • Other respiratory conditions where mucus clearance is impaired

Mechanism of Action

Bromhexine works through several pathways:

  • It depolymerizes mucopolysaccharides in bronchial secretions, breaking down the long sugar-chain molecules that make mucus thick and sticky, thereby reducing its viscosity.
  • It stimulates the serous cells of the bronchial mucosa to produce thinner, more watery secretions, further diluting the mucus.
  • It enhances the activity of the cilia (tiny hair-like structures) lining the airways that sweep mucus toward the throat for removal.
  • Bromhexine is a prodrug and is metabolized in the body to its active form, ambroxol, which also exerts mucolytic effects.

Dosage

Dosage depends on age, body weight, and formulation. General guidelines are:

  • Adults and adolescents 14 years and older: typically 8–16 mg, three times daily
  • Children aged 6 to 14 years: typically 8 mg, three times daily
  • Children under 6 years: only under medical supervision, usually 4 mg, three times daily

Side Effects

Bromhexine is generally well tolerated. Possible side effects include:

  • Common: Gastrointestinal complaints such as nausea, vomiting, stomach pain, or diarrhea
  • Uncommon: Headache, dizziness, mild fever
  • Rare: Allergic reactions such as skin rash, itching, or facial swelling
  • Very rare: Severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis) – seek immediate medical attention if these occur

Contraindications and Warnings

Bromhexine should not be used in cases of:

  • Known hypersensitivity to the active substance
  • Severe liver disease (consult a physician before use)
  • Gastric ulcers – use with caution

During pregnancy, especially in the first trimester, and during breastfeeding, bromhexine should only be taken on explicit medical advice.

Drug Interactions

Bromhexine has been shown to enhance the distribution of certain antibiotics (e.g., amoxicillin, doxycycline, erythromycin) into lung tissue, a property that may be used beneficially in clinical practice. Other clinically significant interactions have not been sufficiently documented.
