What is the CAGE Test?

The CAGE Test is a short, standardized screening tool designed to quickly identify problematic alcohol use or alcohol dependence. The acronym CAGE stands for the key words in its four questions: Cut down, Annoyed, Guilty, and Eye-opener. Developed in 1968 by John Ewing, it is one of the most widely used screening instruments in addiction medicine and general practice worldwide.

The Four CAGE Questions

Patients are asked the following four questions:

C (Cut down): Have you ever felt you should cut down on your drinking?
A (Annoyed): Have you ever been annoyed by people criticizing your drinking?
G (Guilty): Have you ever felt guilty or bad about your drinking?
E (Eye-opener): Have you ever had a drink first thing in the morning to steady your nerves or get the day started?

Scoring and Interpretation

Each Yes answer scores one point. The total score ranges from 0 to 4:

0 points: No indication of problematic alcohol use.
1 point: Possible indication; further assessment is recommended.
2 or more points: Clinically significant indication of alcohol misuse or dependence; a more thorough diagnostic evaluation is strongly recommended.

A threshold score of 2 or more is widely regarded in clinical literature as a positive screening result, indicating potentially harmful alcohol use.
