Carbapenems are a class of broad-spectrum antibiotics belonging to the beta-lactam antibiotic group. They are considered among the most potent antibiotics available and are primarily used for severe, life-threatening infections caused by multidrug-resistant bacteria. Well-known representatives of this class include imipenem, meropenem, ertapenem, and doripenem.
Carbapenems work by inhibiting bacterial cell wall synthesis. They bind to penicillin-binding proteins (PBPs), which are essential for the construction and stability of the bacterial cell wall. This binding blocks cell wall production, ultimately causing the bacteria to die (bactericidal effect). Carbapenems are stable against most bacterial beta-lactamases – enzymes that can render many other antibiotics ineffective.
Carbapenems are generally used as last-resort or reserve antibiotics, meaning they are only deployed when other antibiotics are no longer effective. Typical areas of use include:
Carbapenems are administered exclusively intravenously (into a vein) or intramuscularly (into a muscle), as they are not adequately absorbed from the gastrointestinal tract when taken orally. The exact dosage depends on the specific agent, the severity of infection, body weight, and the renal function of the patient. Treatment takes place in an inpatient setting under medical supervision.

The increasing spread of carbapenem-resistant bacteria (e.g., carbapenem-resistant Enterobacteriaceae, CRE) is a serious global health concern. This resistance arises mainly through the production of carbapenemases – enzymes that break down carbapenems and render them ineffective. Responsible use of these antibiotics through antibiotic stewardship programs is therefore critical to slowing the development of further resistance.
