What is Carnosol?
Carnosol is a polyphenolic diterpene compound naturally found in medicinal herbs such as rosemary (Rosmarinus officinalis) and sage (Salvia officinalis). It belongs to the group of abietane diterpenols and is closely related to carnosic acid, another bioactive compound found in these plants. Carnosol contributes to the characteristic biological activity of rosemary and sage and is being actively investigated by modern researchers for its potential health-promoting properties.

Carnosol is primarily found in the following plants:
- Rosemary (Rosmarinus officinalis) – one of the richest natural sources
- Sage (Salvia officinalis)
- Other plants of the Lamiaceae (mint) family
In the food industry, rosemary extracts containing carnosol are used as natural antioxidants for preservation purposes. In the European Union, rosemary extract is approved as the food additive E 392.

Antioxidant Activity
Carnosol is a potent antioxidant. It neutralizes free radicals and protects cells from oxidative stress, which is associated with aging processes and various chronic diseases. It also inhibits lipid peroxidation, meaning the oxidative degradation of fatty molecules in cell membranes.
Anti-inflammatory Activity
Carnosol modulates key inflammatory signaling pathways, including:
- Inhibition of the NF-κB signaling pathway (Nuclear Factor kappa-light-chain-enhancer of activated B cells), a central regulator of the inflammatory response
- Reduction of pro-inflammatory cytokines such as TNF-α and IL-6
- Inhibition of cyclooxygenase-2 (COX-2), a key enzyme involved in inflammation
Anti-cancer Properties
Numerous laboratory and preclinical studies suggest that carnosol may have cancer-protective properties. The following mechanisms have been described:
- Promotion of programmed cell death (apoptosis) in tumor cells
- Inhibition of cell proliferation (uncontrolled cell growth)
- Modulation of signaling pathways such as the Nrf2 pathway, which regulates the cellular stress response
- Anti-angiogenic effects (inhibition of new blood vessel formation in tumors)
So far, these effects have been observed mainly in cell cultures and animal models. Clinical studies in humans are still largely lacking.
Traditional Use
Rosemary and sage have been used in folk medicine for centuries – among other purposes, to support digestion, enhance memory, and reduce inflammation. Carnosol is considered one of the key bioactive compounds responsible for these traditional uses.
Application and Dosage
Carnosol is not yet used as an isolated active ingredient in standardized pharmaceutical preparations. It is primarily consumed as part of rosemary extracts or sage extracts, either through the diet (fresh or dried herbs) or via herbal dietary supplements. No official recommended daily dose for carnosol as a standalone compound currently exists. Intake through a herb-rich diet is generally considered safe.
Safety and Potential Risks
In amounts typically consumed through food, carnosol is considered well-tolerated and safe. However, the following points should be noted:
- At very high concentrations, rosemary extracts may show pro-oxidative effects – though this is not relevant at normal dietary intake levels.
- Pregnant and breastfeeding women should avoid high-dose rosemary extracts.
- Interactions with medications (e.g., blood thinners) are possible with high-dose extracts.
Scientific Evidence
Research on carnosol is still predominantly in the preclinical phase. While cell and animal studies provide promising results, clinical trials in humans remain scarce. Carnosol is therefore regarded as an interesting bioactive natural compound with high research potential, but specific medical recommendations for use in human medicine cannot yet be made.
