What is a Cast Metal Partial Denture?

The foundation of the prosthesis is a precision-cast metal framework, typically made from a chrome-cobalt alloy, which provides exceptional strength while remaining thin and lightweight. Artificial teeth and a gum-coloured acrylic base are then attached to this framework to restore both function and aesthetics.

A cast metal partial denture is recommended in the following situations:

• Multiple teeth are missing, but sufficient healthy teeth remain to serve as anchor teeth (abutments).
• Fixed prosthetic solutions such as bridges or implants are not feasible or not desired by the patient.
• A cost-effective yet durable and stable solution is required.
• Larger gaps need to be restored that would place excessive strain on a conventional dental bridge.

Structure and Components

The cast metal partial denture consists of several key components that work together to ensure fit, function, and comfort:

• Metal framework: The precision-cast base structure made from a biocompatible alloy (typically chrome-cobalt). It is strong, thin, and custom-fitted to the individual anatomy of the mouth.
• Clasps (retentive elements): Metal arms that wrap around the abutment teeth to hold the denture in place. They are designed to be as discreet as possible while providing reliable retention.
• Denture saddle: The pink-coloured acrylic base that mimics the appearance of natural gum tissue and rests on the jawbone ridge.
• Artificial teeth: Replacement teeth made from acrylic resin or ceramic, matched in shape and colour to the remaining natural teeth.
• Connector elements: The major connector (palatal plate or bar in the upper jaw, lingual bar in the lower jaw) links the different parts of the framework and distributes chewing forces evenly.

Manufacturing – How is a Cast Metal Partial Denture Made?

The fabrication of a cast metal partial denture requires several clinical and laboratory steps:

1. Examination and planning: The dentist evaluates the oral health status, assesses the remaining teeth, and develops a treatment plan.
2. Impressions: Precise impressions of the upper and lower jaws are taken.
3. Model fabrication: A plaster cast is created in the dental laboratory based on the impressions.
4. Design and casting: The metal framework is designed on the model, waxed up, and then cast in metal.
5. Try-in and adjustment: The framework is tried in the patient's mouth and adjusted as needed.
6. Completion and fitting: The artificial teeth and acrylic saddle are attached to the framework, and the finished denture is inserted.

• Meticulous oral hygiene around the abutment teeth is essential to prevent tooth decay
• Generally less comfortable than fixed prosthetic options

Care and Cleaning

Proper maintenance of a cast metal partial denture is essential for longevity and oral health:

• Clean the denture daily with a soft-bristled brush and a dedicated denture cleanser. Avoid regular toothpaste, as it can be too abrasive for the denture materials.
• Remove the denture at night and store it in water or a denture-soaking solution to allow the gum tissue to recover.
• Continue to brush the remaining natural teeth, gums, and tongue thoroughly and use dental floss regularly.
• Attend regular dental check-ups at least once a year to have the fit assessed and adjusted if necessary.

Costs and Insurance Coverage

In Germany, the cast metal partial denture is classified as a standard dental care option under the statutory health insurance system (GKV). Patients receive a fixed subsidy based on their dental findings. The subsidy may be increased if the patient can demonstrate regular dental attendance via a dental bonus booklet (Bonusheft). Any remaining costs vary depending on the dental practice and individual preferences. Patients with private insurance or those paying out of pocket receive an individual cost estimate.
