What is Cefpodoxime?

Cefpodoxime is a broad-spectrum antibiotic of the third-generation cephalosporin class. It is administered orally as a prodrug called cefpodoxime proxetil, which is converted into the active compound cefpodoxime after absorption in the intestine. Cephalosporins belong to the larger class of beta-lactam antibiotics and are effective against a wide range of bacterial pathogens.

Indications

Cefpodoxime is used to treat various bacterial infections, including:

  • Respiratory tract infections: acute bronchitis, community-acquired pneumonia
  • ENT infections: otitis media (middle ear infection), sinusitis, tonsillitis, pharyngitis
  • Urinary tract infections: uncomplicated cystitis (bladder infection) and pyelonephritis (kidney infection)
  • Skin and soft tissue infections: uncomplicated cases
  • Gonorrhea: uncomplicated urogenital gonorrhea

Mechanism of Action

Cefpodoxime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which are essential enzymes involved in constructing and maintaining the structural integrity of the bacterial cell wall. By blocking these proteins, cefpodoxime weakens the cell wall, ultimately causing the bacterial cell to lyse (break apart) and die. This makes cefpodoxime a bactericidal (bacteria-killing) agent.

Its spectrum of activity covers many gram-positive organisms such as Streptococcus pneumoniae and Staphylococcus aureus (non-MRSA), as well as gram-negative organisms including Haemophilus influenzae, Moraxella catarrhalis, and members of the Enterobacteriaceae family.

Dosage and Administration

Cefpodoxime is available as tablets for adults and as an oral suspension for children. The dose depends on the type and severity of the infection and, in children, on body weight. Typical adult doses range from 100 to 200 mg twice daily. It is recommended to take cefpodoxime with food, as this improves the bioavailability of the drug.

Treatment duration is generally 5 to 14 days depending on the indication and clinical response. Patients should complete the full course of treatment even if symptoms improve earlier, in order to prevent antibiotic resistance.

Side Effects

Like all antibiotics, cefpodoxime may cause side effects. Common side effects include:

  • Gastrointestinal complaints: nausea, vomiting, diarrhea, abdominal pain
  • Headache
  • Disruption of gut flora (dysbiosis)

Less common but notable side effects include:

  • Allergic reactions (rash, itching, and in rare cases severe hypersensitivity reactions)
  • Clostridioides difficile-associated diarrhea (pseudomembranous colitis)
  • Elevated liver enzyme levels
  • Blood count changes

Patients with a known penicillin allergy should inform their doctor before taking cefpodoxime, as cross-reactivity with cephalosporins is possible in approximately 1-2% of cases.

Contraindications and Interactions

Cefpodoxime is contraindicated in patients with known hypersensitivity to cephalosporins. Caution is required in patients with renal impairment (dose adjustment may be needed) and in pregnant or breastfeeding women (use only after careful benefit-risk assessment by a healthcare professional).

Important drug interactions include:

  • Antacids and H2-blockers: Reduce the absorption of cefpodoxime by increasing gastric pH
  • Probenecid: Inhibits renal excretion of cefpodoxime, leading to increased plasma levels
  • Anticoagulants (e.g., warfarin): Possible enhancement of anticoagulant effect
