What is Chemerin?

Chemerin is an adipokine – a signaling protein produced mainly by adipose (fat) tissue and the liver. It is a protein of approximately 16 kilodaltons that fulfills multiple roles in the body: it regulates immune responses, influences fat metabolism, and contributes to inflammatory processes. Chemerin circulates in the bloodstream as an inactive precursor called pro-chemerin and is converted into its active form by specific enzymes. Once activated, it binds to dedicated receptors – most notably CMKLR1 (chemokine-like receptor 1) – and exerts its effects in various tissues and cell types.

Biological Functions

Immunological Role

Chemerin plays a central role in regulating the immune system. It acts as a chemokine, meaning it attracts specific immune cells – particularly dendritic cells, macrophages, and natural killer cells – to sites of inflammation. Through this mechanism, chemerin is involved in both initiating and sustaining immune responses throughout the body.

Metabolic Role

As a metabolic regulator, chemerin influences the formation of fat cells (adipogenesis), insulin sensitivity, and glucose utilization. Elevated chemerin levels in the blood have been linked to reduced insulin action (insulin resistance) and impaired glucose metabolism. Chemerin also modulates the release of other adipokines such as adiponectin and leptin, further underscoring its role in the metabolic network.

Obesity and Metabolic Syndrome

In individuals with obesity and metabolic syndrome, blood chemerin levels are frequently elevated. Scientific studies show an association between high chemerin concentrations and the development of type 2 diabetes, hypertension (high blood pressure), and abnormal blood lipid levels (dyslipidemia). Chemerin is therefore considered a potential biomarker for metabolic risk assessment.

Inflammatory Diseases

Elevated chemerin levels have been observed in various inflammatory conditions, including rheumatoid arthritis, psoriasis, and inflammatory bowel disease. In the joint fluid of patients with rheumatoid arthritis, chemerin has been detected at increased concentrations, suggesting a direct involvement in joint inflammation.

Cardiovascular Disease

Emerging research suggests that chemerin may also be implicated in the development of atherosclerosis (hardening of the arteries) and other cardiovascular conditions. High chemerin levels are associated with increased inflammatory activity in vessel walls and a heightened cardiovascular risk profile.

Diagnosis and Clinical Relevance

Chemerin levels can be measured using a standard blood test (ELISA). Reference ranges may vary depending on the laboratory and measurement method used. In clinical practice, chemerin measurement is currently used primarily in research settings to better understand metabolic and inflammatory diseases. A standardized routine diagnostic application does not yet exist, but ongoing research may establish its role as a clinical tool – especially in patients with obesity, type 2 diabetes, or rheumatic conditions.

Therapeutic Approaches and Outlook

Given its involvement in numerous biological processes, chemerin is being intensively studied as a potential therapeutic target. Approaches aimed at blocking the chemerin receptor CMKLR1 are being investigated in preclinical studies. Additionally, research shows that weight loss, regular physical activity, and a balanced diet can lower chemerin levels, thereby positively influencing metabolism and reducing inflammatory activity in the body.
