Biological Basis of Chemokines

Chemokines (from chemotactic cytokines) represent a family of more than 50 known proteins, classified into four structural subgroups:

Inflammatory Diseases

In chronic inflammatory conditions such as rheumatoid arthritis, Crohn disease, or systemic lupus erythematosus, characteristic chemokine patterns can be detected. These patterns help assess disease activity and monitor the response to treatment.

Infectious Diseases

During bacterial, viral, or parasitic infections, the chemokine profile typically changes significantly. Specific patterns can indicate the type of pathogen or the severity of the infection. For example, CXCL10 (IP-10) is markedly elevated in severe viral infections such as COVID-19 or influenza.

Oncology

In cancer research and therapy, chemokines influence the tumor microenvironment, may promote or inhibit metastasis, and can determine the success of immunotherapy. The chemokine profile of a cancer patient can provide prognostic and predictive information.

Transplant Medicine

After organ or stem cell transplantation, the chemokine profile is used for the early detection of rejection reactions or graft-versus-host disease (GvHD).

Autoimmune and Neurological Diseases

Therapeutic Relevance

Understanding the chemokine profile has led to the development of novel therapeutic approaches. Chemokine receptor antagonists selectively block specific chemokine-receptor interactions and are used across various indications:

  • Maraviroc: A CCR5 antagonist used in HIV therapy, as CCR5 serves as a co-receptor for HIV entry into immune cells.
  • Additional antagonists are under clinical development for indications such as rheumatoid arthritis, inflammatory bowel disease, and cancer.
