CHIVA stands for the French term Cure Conservatrice et Hemodynamique de l'Insuffisance Veineuse en Ambulatoire, which translates to: Conservative and Hemodynamic Treatment of Venous Insufficiency in an Outpatient Setting. It is a minimally invasive, vein-preserving procedure for treating varicose veins (varices) that is based on a precise analysis of blood flow patterns in the leg veins.
Unlike classical surgical methods such as vein stripping, which involves removing entire venous segments, the CHIVA method aims to interrupt only the hemodynamically relevant points of insufficiency. This restores normal blood flow and allows the diseased vein segments to gradually regress without being completely removed.
The CHIVA method is based on a thorough hemodynamic analysis of the venous system. Before the procedure, a detailed duplex ultrasound examination is performed to identify the exact locations where blood is flowing in the wrong direction through the wrong veins -- a phenomenon known as venous reflux. These locations are referred to as insufficiency points.
Through targeted, small surgical interventions at these insufficiency points, the abnormal blood flow is interrupted. Without the excess pressure from the deep venous system, the superficial varicose veins gradually regress over time. The deep venous system and healthy portions of the superficial veins are preserved.

The procedure is not suitable for patients with active deep vein thrombosis or severe anatomical changes to the deep venous system.
Before the intervention, a comprehensive duplex ultrasound examination is performed. This imaging technique provides precise information about the location, extent, and direction of blood flow in the affected veins. Based on these findings, the physician develops an individualized treatment plan.
The procedure is typically performed on an outpatient basis -- no hospital stay is required -- and is usually carried out under local anesthesia. Only a few very small skin incisions are made to precisely interrupt the identified insufficiency points. Depending on the findings, one or more treatment sessions may be necessary.
Following the procedure, patients are generally advised to wear compression stockings to support the regression of varicose veins and to minimize swelling. Regular follow-up ultrasound examinations are used to monitor the success of the treatment.

Serious complications such as deep vein thrombosis or pulmonary embolism are very rare and occur significantly less often than with more invasive procedures.
