What is Clemizole Benzylpenicillin?

Clemizole benzylpenicillin is an antibiotic belonging to the penicillin group. It is classified as a depot penicillin, in which the active substance benzylpenicillin (penicillin G) is combined with clemizole -- a first-generation antihistamine -- to form a salt. This combination allows the active ingredient to be slowly released from the muscle tissue after intramuscular injection, resulting in a prolonged duration of action compared to standard penicillin G formulations.

Indications

Clemizole benzylpenicillin is used to treat infections caused by penicillin-susceptible bacteria. Typical indications include:

  • Syphilis and other infections caused by Treponema pallidum
  • Streptococcal infections, such as scarlet fever or erysipelas
  • Pneumococcal respiratory tract infections
  • Prophylaxis of rheumatic fever
  • Other infections caused by gram-positive and gram-negative organisms responsive to penicillin G

Mechanism of Action

The antibacterial component of this medication is benzylpenicillin (penicillin G). It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), which are enzymes responsible for cross-linking the peptidoglycan layer of the bacterial cell wall. By blocking these enzymes, cell wall synthesis is disrupted, leading to bacterial lysis and death. Clemizole functions solely as a carrier substance; its poor water solubility prolongs the absorption time of the penicillin salt from the injection site, creating the depot effect.

Dosage and Administration

Clemizole benzylpenicillin is administered exclusively by intramuscular injection and must never be given intravenously, as this can lead to serious adverse reactions. Dosage depends on the type and severity of the infection, as well as the age and body weight of the patient.

  • Adults: Typical doses range from 1.2 to 2.4 million units (IU) per injection, depending on the indication.
  • Children: Dosage is calculated based on body weight and determined by the treating physician.
  • Injection intervals of 1 to 3 days are common, as the depot formulation allows for less frequent dosing.

The exact dosage is always determined individually by a physician.

Side Effects

Like all medicines, clemizole benzylpenicillin can cause side effects. Common and possible adverse effects include:

  • Hypersensitivity reactions: ranging from skin rashes to life-threatening anaphylaxis in patients with known penicillin allergy
  • Local reactions: pain, redness, and swelling at the injection site
  • Gastrointestinal complaints: nausea, diarrhea
  • Neurological reactions: seizures are possible at very high doses (penicillin encephalopathy)
  • Hematological changes: such as hemolytic anemia or neutropenia with long-term use
  • Sedative or antihistaminergic effects from the clemizole component are generally not clinically relevant at therapeutic doses

Contraindications and Interactions

Clemizole benzylpenicillin must not be used in patients with:

  • Known allergy to penicillins or other beta-lactam antibiotics
  • Intravenous or intra-arterial administration is absolutely contraindicated

Important drug interactions include:

  • Methotrexate: Penicillins can reduce the renal excretion of methotrexate, potentially increasing its toxicity.
  • Probenecid: Delays the excretion of penicillin, thereby increasing plasma concentrations.
  • Bacteriostatic antibiotics (e.g., tetracyclines, chloramphenicol): May reduce the bactericidal effect of penicillin.
