What is Complex III?

Complex III, also known as ubiquinol-cytochrome c oxidoreductase or the cytochrome bc1 complex, is a central enzyme complex of the mitochondrial respiratory chain (electron transport chain). It is embedded in the inner mitochondrial membrane and plays a crucial role in cellular energy production. The respiratory chain consists of five major complexes (Complex I through V), with Complex III forming the third link in this chain.

Mechanism of Action

The primary function of Complex III is to transfer electrons from reduced ubiquinol (CoQH2) to the protein cytochrome c. This process proceeds via the so-called Q cycle:

  • Ubiquinol (reduced coenzyme Q10) donates two electrons.
  • One electron is transferred to cytochrome c, which carries it to Complex IV.
  • The second electron is used to reduce ubiquinone (the oxidized form) back to ubiquinol, maintaining the cycle.
  • Simultaneously, protons (H+) are pumped from the mitochondrial matrix into the intermembrane space, generating an electrochemical gradient.

This proton gradient drives the ATP synthase (Complex V) and enables the production of adenosine triphosphate (ATP), the universal energy carrier of the cell.

Structure of Complex III

Complex III is a dimer (composed of two identical subunits) and consists of 11 subunits in humans. The most important structural and functional components include:

  • Cytochrome b: The only protein encoded by the mitochondrial genome; contains two heme-b groups (bL and bH).
  • Cytochrome c1: A nuclear-encoded protein with a heme-c1 group that transfers electrons to cytochrome c.
  • Rieske iron-sulfur protein (ISP): Contains an iron-sulfur center and is critical for electron transport.

Clinical Significance

Dysfunction of Complex III can lead to serious diseases, as cells can no longer produce sufficient energy. Tissues with high energy demands such as muscles, the heart, and the brain are particularly affected.

Complex III Deficiency

Complex III deficiency (also called cytochrome bc1 complex deficiency) is a rare mitochondrial disorder usually caused by mutations in the genes encoding the subunits or assembly factors of the complex. Common symptoms include:

  • Muscle weakness and wasting (myopathy)
  • Lactic acidosis (elevated lactic acid levels in the blood)
  • Encephalopathy (brain dysfunction)
  • Cardiomyopathy (heart muscle disease)
  • Liver dysfunction

Oxidative Stress

Complex III is one of the main sources of reactive oxygen species (ROS) production in mitochondria. Impaired function can lead to increased generation of free radicals, causing oxidative stress, which has been linked to age-related diseases, neurodegenerative conditions, and cancer.

Diagnosis

Diagnosis of Complex III deficiency is established through:

  • Enzyme activity measurement: Analysis of ubiquinol-cytochrome c oxidoreductase activity in muscle biopsies or other tissues.
  • Genetic testing: Sequencing of the mitochondrial and nuclear genomes to identify mutations.
  • Laboratory parameters: Elevated lactate and pyruvate levels in blood and cerebrospinal fluid (CSF).
  • Imaging: Brain MRI to detect structural changes.

Treatment

A curative therapy for Complex III deficiency is not yet available. Treatment is symptomatic and may include:

  • Coenzyme Q10 (ubiquinone): Supplementation to support the electron transport chain.
  • Riboflavin (vitamin B2): May improve mitochondrial function in some cases.
  • Antioxidants: To reduce oxidative stress (e.g., vitamin E, vitamin C).
  • Carnitine: To support fatty acid metabolism.
  • Symptomatic treatment of associated conditions such as cardiomyopathy or lactic acidosis.
