The daily blood pressure profile is a method of recording blood pressure over a period of typically 24 hours. A portable monitoring device worn on the upper arm automatically measures blood pressure at regular intervals – more frequently during the day than at night. This method is also known as ambulatory blood pressure monitoring (ABPM).

Unlike a single measurement taken at a clinic, the daily blood pressure profile provides a comprehensive picture of blood pressure fluctuations during everyday activities and sleep, revealing patterns that a single reading might miss.

Why Is the Daily Blood Pressure Profile Important?

Blood pressure is not constant – it changes throughout the day depending on physical activity, stress, sleep, and other factors. A healthy blood pressure typically shows a nighttime drop, referred to as dipping. When this drop is absent, the pattern is called non-dipping, which is associated with an increased cardiovascular risk.

The daily blood pressure profile is especially useful for:

  • Detecting the white coat effect (elevated readings only at the clinic)
  • Identifying masked hypertension (normal readings at the clinic but elevated readings in daily life)
  • Monitoring the effectiveness of blood pressure medications
  • Investigating dizziness, fainting, or significant blood pressure fluctuations
  • Assessing blood pressure in pregnant women, diabetic patients, or those with kidney disease

How Is the Daily Blood Pressure Profile Performed?

The patient is fitted with a portable blood pressure monitor attached via a cuff around the upper arm. The device measures blood pressure every 15 to 30 minutes during the day and every 30 to 60 minutes at night. During the monitoring period, the patient keeps a diary noting activities, meals, sleep times, and any notable events.

After 24 hours, the recorded data is retrieved and analyzed. The physician evaluates:

  • Average blood pressure values during the day and night
  • Nighttime blood pressure reduction (dipping)
  • Morning blood pressure spikes (morning surge)
  • Maximum and minimum blood pressure values

Normal Values for Daily Blood Pressure Monitoring

The reference values for ambulatory blood pressure monitoring differ from those used in clinic measurements. According to current guidelines, the following thresholds indicate elevated blood pressure:

  • Daytime average: above 135/85 mmHg
  • Nighttime average: above 120/70 mmHg
  • 24-hour average: above 130/80 mmHg

These values are lower than the clinic measurement threshold of 140/90 mmHg, as blood pressure in everyday settings is generally expected to be somewhat lower.

Interpretation and Clinical Significance

Analyzing the daily blood pressure profile enables a precise assessment of cardiovascular risk. Specific patterns carry distinct clinical significance:

  • Dipper: Nighttime blood pressure drop of 10–20% – normal, healthy pattern
  • Non-dipper: Nighttime drop of less than 10% – increased risk of heart attack and stroke
  • Extreme dipper: Drop of more than 20% – may indicate overmedication or circulatory issues
  • Reverse dipper (night peaker): Nighttime rise in blood pressure – particularly high cardiovascular risk
