Darolutamide is a medication used to treat prostate cancer. It inhibits the androgen receptor and is indicated for non-metastatic castration-resistant prostate cancer.

What is Darolutamide?

Darolutamide is a second-generation androgen receptor inhibitor used in the treatment of prostate cancer. It belongs to the class of non-steroidal antiandrogens and has been approved by both the European Medicines Agency (EMA) and the U.S. Food and Drug Administration (FDA). It is marketed under the brand name Nubeqa.

Indications

Darolutamide is approved for:

  • Non-metastatic castration-resistant prostate cancer (nmCRPC): in patients at high risk of developing metastases
  • Metastatic hormone-sensitive prostate cancer (mHSPC): in combination with docetaxel and androgen deprivation therapy (ADT)

Mechanism of Action

Darolutamide blocks the androgen receptor (AR) through several mechanisms:

  • It prevents the binding of androgens (male sex hormones such as testosterone and dihydrotestosterone) to the receptor.
  • It inhibits the nuclear translocation of the receptor, meaning it prevents the receptor from entering the cell nucleus.
  • It blocks the activation of androgen-dependent genes that promote tumor growth.

Through this multi-level inhibition, the growth of prostate cancer cells that depend on androgens is effectively suppressed. Compared to other androgen receptor inhibitors, darolutamide shows lower penetration of the blood-brain barrier, which reduces the risk of central nervous system side effects such as seizures.

Dosage and Administration

The recommended dose is 600 mg twice daily (1200 mg per day), taken as tablets with food. Treatment is typically combined with androgen deprivation therapy (ADT), such as a GnRH analogue.

Side Effects

Darolutamide is generally well tolerated. Commonly reported side effects include:

  • Fatigue
  • Pain in the extremities
  • Skin rash
  • Elevated liver enzyme levels (AST, ALT)
  • Neutropenia (reduced white blood cell count)

Serious side effects such as seizures occur less frequently than with other agents in this class, as darolutamide crosses the blood-brain barrier to a lesser extent.

Clinical Studies

The efficacy of darolutamide has been demonstrated in several large clinical trials:

  • ARAMIS Trial: Showed a significant improvement in metastasis-free survival in nmCRPC patients compared to placebo.
  • ARASENS Trial: Demonstrated a significant overall survival benefit in mHSPC patients treated with the combination of darolutamide, docetaxel, and ADT.

Drug Interactions

Darolutamide may interact with various medications, including:

  • CYP3A4 inducers (e.g., rifampicin): may reduce darolutamide blood levels.
  • P-glycoprotein and BCRP inhibitors: may increase drug levels.

Patients should inform their treating physician about all medications and supplements they are taking.
