Things worth knowing about "Death cap mushroom"

The death cap mushroom is one of the most toxic fungi in the world and the leading cause of fatal mushroom poisonings in Europe. Even small amounts can cause severe liver failure.

What is the Death Cap Mushroom?

The death cap mushroom (Amanita phalloides) is the most dangerous poisonous fungus in Europe and is responsible for the majority of fatal mushroom poisonings worldwide. It belongs to the genus Amanita and grows widely in deciduous and mixed forests. Despite its extreme toxicity, it is frequently confused by non-experts with edible mushrooms such as the common button mushroom or the blusher.

Appearance and Identifying Features

The death cap mushroom typically has the following characteristics:

  • Cap: 5–15 cm wide, yellowish-green to olive green, rarely whitish
  • Gills: white, free-standing, not attached to the stem
  • Stem: white to yellowish, with a hanging ring (annulus)
  • Base: characteristic bulbous swelling enclosed in a white sheath (volva)
  • Smell: pleasantly sweet, which increases the danger

The greenish coloration can vary and is not a reliable identifying feature. The volva (the sac-like sheath at the base of the stem) is the most important distinguishing characteristic from harmless mushrooms.

Toxic Compounds and Mechanism of Action

The death cap mushroom contains several highly toxic substances:

  • Amatoxins (particularly α-amanitin): Inhibit the enzyme RNA polymerase II, thereby blocking protein biosynthesis in cells. This leads to massive cell death, primarily in the liver and kidneys.
  • Phallotoxins (e.g., phalloidin): Damage cell membranes, but contribute less to systemic poisoning as they are poorly absorbed through the intestine.
  • Virotoxins: Structurally similar to phallotoxins, also membrane-damaging.

As little as 0.1 mg of amanitin per kilogram of body weight (equivalent to approximately 20–30 g of fresh mushroom) can be lethal for an adult. A single mushroom contains enough toxin to kill a human being.

Symptoms of Poisoning

Poisoning by the death cap mushroom typically progresses through four phases:

Phase 1: Latent Phase (6–24 hours)

After ingestion, no symptoms appear initially. This deceptive symptom-free period often leads those affected to delay seeking medical attention.

Phase 2: Gastrointestinal Phase (24–48 hours)

Severe gastrointestinal symptoms develop: intense nausea, vomiting, colicky abdominal pain, and cholera-like diarrhea with massive fluid loss.

Phase 3: Apparent Recovery Phase (48–72 hours)

Gastrointestinal symptoms appear to subside. However, serious damage to the liver and kidneys is already occurring internally, detectable through laboratory tests.

Phase 4: Organ Failure (after 72 hours)

Severe liver failure (jaundice, bleeding tendency, hepatic encephalopathy) and kidney failure develop. Without timely treatment or liver transplantation, the poisoning can be fatal.

Diagnosis

Diagnosis is based on:

  • Patient history (mushroom consumption within the past 24–48 hours)
  • Blood tests: liver enzymes (AST, ALT), bilirubin, coagulation parameters (prothrombin time/INR), kidney function markers
  • Toxicological detection of amatoxins in urine
  • Mycological identification of mushroom remnants or vomit

If poisoning by the death cap mushroom is suspected, emergency services or a poison control center must be contacted immediately. Every suspected case is a medical emergency.

Treatment

There is no specific antidote for amatoxins. Treatment includes:

  • Immediate measures: Toxin elimination (e.g., activated charcoal, if administered shortly after ingestion), intensive care monitoring
  • Silibinin (Legalon SIL): An active compound derived from milk thistle that inhibits the uptake of amatoxins into liver cells – considered the most important pharmacological treatment option
  • N-acetylcysteine: Supports liver regeneration and acts as an antioxidant
  • Penicillin G: May inhibit the binding of amatoxins to liver receptors
  • Intensive care support: Fluid replacement, dialysis for kidney failure, management of coagulation disorders
  • Liver transplantation: As a last resort in cases of severe, irreversible liver failure

Risk of Confusion

The death cap mushroom is frequently confused with:

  • Common button mushroom (Agaricus bisporus) – the button mushroom has brownish-pink gills and no volva
  • The blusher (Amanita rubescens) – turns red when cut or bruised
  • Field mushroom (Agaricus campestris)
  • Destroying angel species (Amanita virosa, Amanita verna) – these are also deadly poisonous

Important: When in doubt, never eat a mushroom. Cooking, pickling, or drying does not render the death cap mushroom safe. The toxins are heat-stable.

Prevention

To prevent poisoning:

  • Only collect mushrooms you can identify with absolute certainty
  • Consult a mycologist or certified mushroom adviser if in doubt
  • Do not accept mushrooms from unknown persons
  • Supervise children closely – they are at even greater risk due to their lower body weight
  • In case of suspected poisoning, immediately call emergency services or a poison control center
