Delayed Onset Muscle Soreness (DOMS) refers to the muscle pain, stiffness, and tenderness that typically develops 12 to 24 hours after unaccustomed or strenuous physical activity, peaking between 24 and 72 hours post-exercise. Unlike acute muscle pain felt during exercise, DOMS is a delayed response and is considered a normal part of the muscle adaptation process.
DOMS is primarily triggered by eccentric muscle contractions, which occur when a muscle lengthens under tension. Examples include walking downhill, lowering weights in a controlled manner, or the downward phase of a push-up. Key contributing factors include:
At the cellular level, the mechanical overload causes micro-tears in muscle fibers. This triggers a local inflammatory response, releasing immune cells and inflammatory mediators such as prostaglandins and cytokines. These substances sensitize pain receptors within the muscle, producing the characteristic soreness.
The symptoms of DOMS are distinct and generally easy to differentiate from acute injury:
Symptoms typically resolve completely within 3 to 7 days without treatment.
DOMS is a clinical diagnosis based on the history of recent physical activity and the characteristic pattern of delayed-onset symptoms. No special tests are usually required. It is important to distinguish DOMS from:
While there is no treatment that completely prevents DOMS, several strategies can ease symptoms and support recovery:
DOMS indicates that the muscle has been exposed to a new stimulus and is undergoing adaptation. The repeated-bout effect describes the well-documented phenomenon that performing the same or similar exercise a second time results in significantly less DOMS, as the muscle adapts. Importantly, DOMS is not a reliable indicator of training quality or muscle growth. Training programs should be based on principles of progressive overload rather than on the degree of post-exercise soreness.
