Differential diagnostics refers to the systematic clinical process by which healthcare professionals compare and evaluate multiple diseases or conditions that may share similar symptoms or clinical findings. The goal is to identify the most likely or confirmed diagnosis by progressively ruling out less probable causes. This process is a cornerstone of clinical medicine and is applied across virtually every medical specialty.
The result of this process is referred to as a differential diagnosis -- a ranked list of possible conditions that could explain the symptoms. Through further testing, history-taking, and examination, this list is progressively narrowed down.
Differential diagnostics is essential in clinical practice because many diseases present with overlapping symptoms. For example, chest pain can be caused by a heart attack, pneumonia, pleuritis, or a muscle strain. Without a structured differential diagnostic approach, there is a significant risk of misdiagnosis, which can lead to incorrect or delayed treatment with serious consequences for the patient.
The process begins with a thorough medical history. The clinician asks the patient about the nature, onset, duration, and severity of symptoms, as well as previous illnesses, current medications, and family history. This information forms the foundation for generating an initial list of possible diagnoses.
The next step is a targeted physical examination, in which the clinician looks for specific clinical signs that may confirm or exclude certain conditions.
