What is Drug Sensitivity?

Drug sensitivity refers to an increased responsiveness of an individual to one or more medications. People with drug sensitivity experience stronger-than-expected reactions to medicines – often at normal or even reduced doses. This can trigger adverse drug reactions (ADRs) or toxic effects without necessarily involving a classic allergic immune response.

Drug sensitivity differs from a drug allergy in that it is not necessarily immune-mediated. Instead, pharmacological, genetic, or metabolic factors are usually responsible.

Causes

The causes of drug sensitivity are multifaceted and may include the following factors:

  • Genetic variants: Variations in genes responsible for drug metabolism (e.g., CYP450 enzymes) can lead to slower breakdown of a medication, causing it to accumulate in the body.
  • Age: Older adults often have reduced kidney and liver function, which slows the elimination of drugs.
  • Underlying conditions: Liver or kidney disease impairs the excretion of active substances.
  • Body weight and composition: Low body weight or an altered ratio of fat to muscle tissue affects how drugs are distributed throughout the body.
  • Drug interactions: Taking multiple medications simultaneously can increase sensitivity to individual substances.
  • Hormonal changes: Pregnancy or thyroid disorders can alter the response to certain medications.

Symptoms

The symptoms of drug sensitivity depend largely on the medication involved. Common signs include:

  • Dizziness, nausea, or vomiting even at low doses
  • Cardiac arrhythmias or changes in blood pressure
  • Skin reactions such as redness, itching, or rash
  • Drowsiness, confusion, or difficulty concentrating (especially with sedatives or pain medications)
  • Increased bleeding tendency (e.g., with heightened sensitivity to anticoagulants)
  • Gastrointestinal complaints

In severe cases, life-threatening reactions such as anaphylactic shock, serious cardiac arrhythmias, or organ failure may occur.

Diagnosis

The diagnosis of drug sensitivity is usually established through a combination of:

  • Medical history: A thorough review of medication use and any reactions that have occurred
  • Pharmacogenetic testing: Genetic tests analyzing metabolic enzymes (e.g., CYP2D6, CYP2C19) to assess individual drug metabolism capacity
  • Laboratory tests: Measurement of drug levels in the blood (therapeutic drug monitoring)
  • Provocation tests: In a controlled medical setting, the medication may be administered in gradually increasing doses to observe the response

Treatment and Management

When drug sensitivity is identified, several management strategies are available:

  • Dose adjustment: The medication dose is individually reduced to prevent adverse reactions.
  • Switching to alternative agents: A drug with a similar therapeutic effect but a different metabolic pathway may be used instead.
  • Close monitoring: Regular surveillance of laboratory values and clinical parameters.
  • Medication plan: A structured medication plan helps identify and avoid interactions.
  • Pharmacogenetically guided therapy: Based on genetic test results, treatment can be individually optimized – an approach known as precision medicine.

At-Risk Groups

Certain populations are particularly prone to drug sensitivity:

  • Older adults (over 65 years of age)
  • Newborns and young children
  • Individuals with liver or kidney disease
  • People with genetically determined metabolic differences (e.g., poor metabolizers)
  • Individuals taking multiple medications simultaneously (polypharmacy)
