What is Dulaglutide?

Dulaglutide is a prescription medication belonging to the class of GLP-1 receptor agonists (glucagon-like peptide-1 receptor agonists). It is marketed under the brand name Trulicity and is approved for the treatment of type 2 diabetes mellitus in adults. Dulaglutide mimics the action of the naturally occurring hormone GLP-1, which is released after meals and plays a key role in regulating insulin secretion.

Indication

Dulaglutide is used when lifestyle modifications and other antidiabetic medications (e.g., metformin) are insufficient to adequately control blood sugar levels. It can be used as monotherapy or in combination with other glucose-lowering agents. Additionally, dulaglutide is approved to reduce the risk of major cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.

Mechanism of Action

Dulaglutide binds to the GLP-1 receptor on pancreatic beta cells and exerts the following effects:

  • Stimulation of insulin secretion: Dulaglutide stimulates the pancreas to release insulin in a glucose-dependent manner, which means it only acts when blood sugar is elevated, minimizing the risk of hypoglycemia.
  • Inhibition of glucagon secretion: It suppresses glucagon, a hormone that would otherwise trigger glucose release from the liver.
  • Slowing of gastric emptying: Food moves more slowly from the stomach into the intestine, prolonging the feeling of fullness after meals.
  • Body weight reduction: Reduced appetite and improved metabolic regulation can lead to meaningful weight loss over time.

Dosage and Administration

Dulaglutide is administered once weekly as a subcutaneous injection (under the skin), typically into the abdomen, thigh, or upper arm. It is available in a prefilled, single-use injection pen. The recommended starting dose is 0.75 mg per week. The dose can be increased to 1.5 mg per week, and in some cases further up to 3 mg or 4.5 mg per week, depending on individual response and tolerability. The injection can be taken at any time of day, regardless of meals, and the day of the week can be changed if needed.

Side Effects

Dulaglutide is generally well tolerated but may cause side effects:

  • Common: Nausea, vomiting, diarrhea, abdominal pain, reduced appetite – these effects are usually mild to moderate and tend to diminish after the first few weeks of treatment.
  • Occasional: Palpitations, fatigue, dizziness.
  • Rare: Pancreatitis (inflammation of the pancreas), renal impairment.
  • Very rare: Allergic reactions at the injection site or systemic hypersensitivity reactions.

The risk of hypoglycemia (low blood sugar) with dulaglutide alone is low, but increases when used in combination with insulin or sulfonylureas.

Contraindications

Dulaglutide should not be used in patients with:

  • Known hypersensitivity to the active substance
  • Type 1 diabetes mellitus or diabetic ketoacidosis
  • Severe gastroparesis or other serious gastrointestinal disorders
  • Personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2 (MEN2)

Dulaglutide is not recommended during pregnancy or breastfeeding, as there is insufficient data on its safety in these populations.

Cardiovascular Benefits

Clinical trials, particularly the landmark REWIND trial, demonstrated that dulaglutide significantly reduces the risk of major adverse cardiovascular events (MACE) – including heart attack and stroke – in adults with type 2 diabetes who have cardiovascular risk factors. This cardioprotective benefit makes dulaglutide a preferred therapeutic choice for high-risk patients.
