Dulaglutide is a prescription medication belonging to the class of GLP-1 receptor agonists (glucagon-like peptide-1 receptor agonists). It is marketed under the brand name Trulicity and is approved for the treatment of type 2 diabetes mellitus in adults. Dulaglutide mimics the action of the naturally occurring hormone GLP-1, which is released after meals and plays a key role in regulating insulin secretion.
Dulaglutide is used when lifestyle modifications and other antidiabetic medications (e.g., metformin) are insufficient to adequately control blood sugar levels. It can be used as monotherapy or in combination with other glucose-lowering agents. Additionally, dulaglutide is approved to reduce the risk of major cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
Dulaglutide binds to the GLP-1 receptor on pancreatic beta cells and exerts the following effects:
Dulaglutide is administered once weekly as a subcutaneous injection (under the skin), typically into the abdomen, thigh, or upper arm. It is available in a prefilled, single-use injection pen. The recommended starting dose is 0.75 mg per week. The dose can be increased to 1.5 mg per week, and in some cases further up to 3 mg or 4.5 mg per week, depending on individual response and tolerability. The injection can be taken at any time of day, regardless of meals, and the day of the week can be changed if needed.

The risk of hypoglycemia (low blood sugar) with dulaglutide alone is low, but increases when used in combination with insulin or sulfonylureas.

Dulaglutide is not recommended during pregnancy or breastfeeding, as there is insufficient data on its safety in these populations.
Clinical trials, particularly the landmark REWIND trial, demonstrated that dulaglutide significantly reduces the risk of major adverse cardiovascular events (MACE) – including heart attack and stroke – in adults with type 2 diabetes who have cardiovascular risk factors. This cardioprotective benefit makes dulaglutide a preferred therapeutic choice for high-risk patients.
