What is Duloxetine?

Duloxetine is a medication belonging to the class of serotonin-norepinephrine reuptake inhibitors (SNRIs). It is prescribed for the treatment of major depression, generalized anxiety disorder, and a range of chronic pain conditions. It is marketed under brand names such as Cymbalta and is available by prescription only.

Indications

Duloxetine is approved for use in the following conditions:

  • Major depressive disorder (MDD)
  • Generalized anxiety disorder (GAD)
  • Diabetic peripheral neuropathic pain
  • Chronic musculoskeletal pain, including fibromyalgia and chronic lower back pain
  • Stress urinary incontinence in women (approved in some countries)

Mechanism of Action

Duloxetine works by inhibiting the reuptake of two neurotransmitters – serotonin and norepinephrine – in the central nervous system. By increasing the availability of these chemical messengers at the synaptic cleft, duloxetine helps stabilize mood and reduces the perception of pain. Its dual action on both serotonergic and noradrenergic pathways distinguishes it from purely serotonergic antidepressants (SSRIs) and contributes to its effectiveness in pain management.

Dosage

The recommended dose of duloxetine varies by indication:

  • Depression and anxiety: 60 mg once daily (may be increased up to 120 mg/day)
  • Diabetic neuropathic pain: 60 mg once daily
  • Chronic musculoskeletal pain: 30–60 mg once daily

Duloxetine is taken orally as enteric-coated capsules, with or without food. Dose adjustments may be necessary for patients with impaired kidney or liver function.

Side Effects

Common side effects of duloxetine include:

  • Nausea and vomiting
  • Headache
  • Dry mouth
  • Dizziness
  • Insomnia or drowsiness
  • Constipation or diarrhea
  • Increased sweating
  • Sexual dysfunction

Less common but more serious side effects may include:

  • Elevated blood pressure (hypertension)
  • Serotonin syndrome – especially when combined with other serotonergic medications
  • Suicidal thoughts or behavior, particularly in children, adolescents, and young adults early in treatment
  • Liver injury or elevated liver enzymes
  • Discontinuation syndrome upon abrupt withdrawal

Drug Interactions

Duloxetine can interact with several medications:

  • MAO inhibitors: Combination is contraindicated due to the risk of life-threatening serotonin syndrome
  • Other antidepressants (SSRIs, tricyclics): increased risk of serotonin syndrome
  • Anticoagulants (e.g., warfarin): increased risk of bleeding
  • CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin): may significantly increase duloxetine plasma levels

Treatment Considerations

Duloxetine should never be stopped abruptly. Sudden discontinuation can cause withdrawal symptoms such as dizziness, irritability, nausea, and sensory disturbances. The dose should be tapered gradually under medical supervision. During pregnancy and breastfeeding, duloxetine should only be used if the potential benefit clearly outweighs the potential risk, as determined by the treating physician.
