What is Dupilumab?

Dupilumab is a monoclonal antibody belonging to the class of biologic medications. It is used to reduce excessive immune responses that occur in various allergic and inflammatory conditions. Dupilumab is marketed under the brand name Dupixent® by Sanofi and Regeneron. The medication is administered as a subcutaneous injection (under the skin).

Indications – When is Dupilumab Used?

Dupilumab is approved for several conditions in which an overactive type-2 immune response plays a central role:

Atopic dermatitis (eczema): For moderate-to-severe cases where other treatments have not been effective enough.
Asthma: As an add-on therapy for severe, uncontrolled asthma with an eosinophilic phenotype or oral corticosteroid-dependent asthma.
Chronic rhinosinusitis with nasal polyps: To reduce polyp size and improve nasal breathing.
Eosinophilic esophagitis: In adults and adolescents aged 12 years and older.
Prurigo nodularis: A chronic skin condition characterized by intensely itchy nodular lesions.

Mechanism of Action

Dupilumab blocks the shared receptor for the signaling molecules Interleukin-4 (IL-4) and Interleukin-13 (IL-13). These two cytokines play a central role in type-2 inflammation, which underlies many allergic and atopic conditions.

By interrupting IL-4 and IL-13 signaling, dupilumab inhibits the following processes:

Production of Immunoglobulin E (IgE), which triggers allergic reactions
Inflammatory responses in the skin, airways, and mucous membranes
Disruption of the skin barrier, a hallmark feature of atopic dermatitis

Dosage and Administration

The dosage of dupilumab depends on the indication, age, and body weight of the patient. Typical dosing regimens include:

Atopic dermatitis (adults): Loading dose of 600 mg (two injections of 300 mg each), followed by 300 mg every two weeks.
Asthma: 200 mg or 300 mg every two weeks, depending on severity.
Chronic rhinosinusitis with nasal polyps: 300 mg every two weeks.

The medication is administered subcutaneously using a prefilled pen or syringe. After appropriate training by a healthcare professional, patients can self-administer injections at home.

Side Effects

Dupilumab is generally well tolerated. The most common side effects include:

Injection site reactions: Redness, swelling, or pain at the injection site
Conjunctivitis: Inflammation of the eye's conjunctiva, particularly in patients with atopic dermatitis
Herpes simplex infections: Increased occurrence of cold sores in some patients
Headache and dizziness

Serious side effects are rare. Dupilumab must not be used in patients with a known hypersensitivity to dupilumab or any of its excipients.

Treatment Context and Key Features

Dupilumab is typically prescribed when conventional therapies – such as topical corticosteroids or systemic immunosuppressants – have been insufficiently effective or poorly tolerated. As a biologic medication, it requires prescription by a specialist (e.g., dermatologist, allergist, or pulmonologist). The therapy can be used long-term, and many patients report a significant improvement in their quality of life.

Unlike corticosteroids, dupilumab does not broadly suppress the immune system. Instead, it intervenes very selectively in the inflammatory process, which means the risk of systemic infections is lower compared to traditional immunosuppressants.
