What is the Dura Mater?

The dura mater (Latin for hard mother) is the outermost of the three meningeal layers (meninges) that surround and protect the brain and spinal cord. It is also known as the pachymeninx, a term reflecting its tough, thick nature. The two inner meningeal layers -- the arachnoid mater and the pia mater -- are collectively referred to as the leptomeninges.

Composed of dense, collagenous connective tissue, the dura mater is the most resilient of all the meningeal layers. It serves as a mechanical protective barrier for the delicate nerve tissue and plays a role in regulating pressure within the central nervous system.

Anatomical Structure

The dura mater surrounding the brain (dura mater encephali) consists of two layers:

  • Periosteal layer: adheres closely to the inner surface of the skull and functions as the periosteum (bone lining) of the cranial cavity.
  • Meningeal layer: forms the actual meningeal covering and lies against the underlying neural tissue.

Between these two layers run venous blood channels known as dural venous sinuses, through which venous blood drains from the brain into the large veins of the neck (jugular veins).

Along the spinal column, the dura mater forms an elongated tubular structure called the dural sac or dura mater spinalis, which encloses the spinal cord. Between the dural sac and the vertebral canal lies the epidural space, which is used for medical procedures such as epidural anesthesia.

Internal Structures and Reflections

The meningeal layer of the dura mater forms folds known as dural reflections or dural folds at specific locations, which divide the cranial cavity into compartments and help stabilize the brain:

  • Falx cerebri: a sickle-shaped fold that runs between the two cerebral hemispheres.
  • Tentorium cerebelli: a tent-like structure that separates the cerebellum from the cerebrum.
  • Falx cerebelli: a small fold between the two cerebellar hemispheres.
  • Diaphragma sellae: a small membrane that forms a roof over the pituitary gland (sella turcica).

Functions of the Dura Mater

The dura mater serves several critical functions:

  • Mechanical protection: It protects the brain and spinal cord from trauma and mechanical forces.
  • Compartmentalization: Its folds divide the cranial cavity into compartments, preventing uncontrolled displacement of brain tissue.
  • Venous drainage: The dural sinuses channel venous blood from the brain toward the jugular veins.
  • Cerebrospinal fluid circulation: The cerebrospinal fluid (CSF) circulates in the subarachnoid space -- between the arachnoid and pia mater -- and its reabsorption is partly facilitated by structures near the dura mater.

Clinical Relevance

Epidural Hematoma

An epidural hematoma occurs when blood accumulates between the skull and the dura mater, typically following a traumatic head injury. The resulting increase in intracranial pressure can compress the brain and represents a medical emergency.

Subdural Hematoma

A subdural hematoma involves blood collecting between the dura mater and the arachnoid mater. It can occur acutely after trauma or develop chronically, especially in older adults.

Meningitis

Meningitis (inflammation of the meninges) affects the meningeal layers including the dura mater. It is most commonly caused by bacteria or viruses and is a serious medical emergency requiring prompt treatment.

Dural Tear and Duraplasty

During trauma or surgical procedures, the dura mater may be torn or damaged. A dural defect can result in CSF leakage. In such cases, a duraplasty -- surgical repair or reconstruction of the dura mater -- is performed to restore its integrity.

Epidural Anesthesia

In epidural anesthesia, an anesthetic agent is injected into the epidural space -- the area between the vertebral canal and the dura mater spinalis. This procedure is widely used during childbirth and surgical operations.

Lumbar Puncture

During a lumbar puncture (spinal tap), a needle is inserted through the dura mater into the subarachnoid space to collect CSF or administer medications. Precise penetration of the dura mater is essential for this procedure.
