What Is Endurance Performance Analysis?

Endurance performance analysis is a diagnostic procedure used to systematically assess and evaluate a person´s physical endurance capacity. It measures physiological parameters such as heart rate, oxygen uptake, blood lactate concentration, and performance indicators under defined exercise conditions. The analysis is used both in sports performance diagnostics and in medical evaluation of cardiovascular and metabolic health.

Goals of Endurance Performance Analysis

The endurance performance analysis pursues several objectives:

  • Determining individual aerobic capacity (maximal oxygen uptake, VO2max)
  • Identifying exercise thresholds such as the anaerobic threshold or lactate threshold
  • Optimizing training management through individualized heart rate or power zones
  • Assessing health status and cardiovascular risk

Methods and Procedures

Incremental Exercise Test on an Ergometer

In the classic incremental test, workload is progressively increased on a cycle ergometer or treadmill. Blood samples are taken at regular intervals to measure lactate concentration. Lactate is a metabolic byproduct that accumulates in greater quantities during intense exercise. Individual training zones can be derived from the lactate curve.

Cardiopulmonary Exercise Testing (CPET)

Cardiopulmonary exercise testing (CPET), also known as spiroergometry, combines an exercise test with continuous measurement of respiratory gases. Oxygen uptake (VO2) and carbon dioxide output (VCO2) are recorded throughout the test. The highest achievable oxygen uptake is referred to as VO2max and is considered the gold standard for assessing aerobic endurance capacity.

Heart Rate-Based Tests

Simpler methods such as the Cooper test or the Conconi test use heart rate as an indirect indicator of endurance performance. These are less resource-intensive but provide less precise data than lactate or CPET-based assessments.

Key Measurement Parameters

  • VO2max: Maximal oxygen uptake in ml per minute per kilogram of body weight; the most important indicator of aerobic capacity
  • Lactate levels: Blood lactate concentration at different exercise intensities
  • Anaerobic threshold: Exercise intensity at which lactate is produced faster than it can be cleared
  • Heart rate: Resting, exercise, and recovery heart rate as indicators of cardiac efficiency
  • Respiratory exchange ratio (RER): Ratio of CO2 output to O2 uptake; indicates the primary energy substrate being used (fat or carbohydrates)

In cardiology and sports medicine, the analysis is used to assess the exercise tolerance of patients recovering from heart disease, surgery, or prolonged inactivity. It forms the basis for individualized rehabilitation programs.

Risks and Contraindications

Exercise tests conducted as part of endurance performance analysis are generally safe when performed under medical supervision and following appropriate screening for contraindications. Possible contraindications include:

  • Acute myocardial infarction or unstable angina pectoris
  • Severe cardiac arrhythmias
  • Uncontrolled hypertension
  • Acute inflammatory conditions or fever

In these cases, the test must be postponed or conducted under special medical monitoring.

Interpretation of Results

The results of the endurance performance analysis are typically evaluated by a specialist experienced in sports medicine. Values are compared against age- and sex-specific reference ranges to provide a well-founded assessment of performance capacity. Based on the analysis, individualized training or therapy recommendations are developed.
