What Is the Erythrocyte Index?

The erythrocyte index is a collective term for a set of laboratory values that describe the size, hemoglobin content, and hemoglobin concentration of red blood cells (erythrocytes). These values are determined as part of a complete blood count (CBC) and provide clinicians with essential information for diagnosing the type and cause of anemia and other blood-related conditions.

The Main Erythrocyte Indices Explained

MCV – Mean Corpuscular Volume

The MCV indicates the average volume of a single red blood cell, measured in femtoliters (fL). The normal range in adults is typically 80 to 100 fL.

  • Elevated MCV (macrocytosis): Red blood cells are larger than normal – often seen in vitamin B12 deficiency, folate deficiency, or excessive alcohol consumption.
  • Decreased MCV (microcytosis): Red blood cells are smaller than normal – typical of iron deficiency anemia or thalassemia.

MCH – Mean Corpuscular Hemoglobin

The MCH reflects the average amount of hemoglobin contained in a single red blood cell, measured in picograms (pg). The normal range is approximately 28 to 34 pg.

  • Elevated MCH (hyperchromia): Too much hemoglobin per cell, seen in megaloblastic anemia.
  • Decreased MCH (hypochromia): Too little hemoglobin per cell, typical of iron deficiency.

MCHC – Mean Corpuscular Hemoglobin Concentration

The MCHC expresses the average concentration of hemoglobin relative to the cell volume, measured in g/dL. The normal range is 32 to 36 g/dL.

  • Elevated MCHC: May indicate hereditary spherocytosis.
  • Decreased MCHC: Commonly seen in hypochromic, microcytic anemia such as iron deficiency anemia.

RDW – Red Cell Distribution Width

The RDW measures the variation in red blood cell size (anisocytosis). An elevated RDW may indicate a mixed-type anemia or an early nutritional deficiency before other indices become abnormal.

Clinical Use of Erythrocyte Indices

Erythrocyte indices are fundamental to the differential diagnosis of anemia. They help distinguish between different forms:

  • Microcytic, hypochromic anemia: Low MCV and MCH – characteristic of iron deficiency anemia or thalassemia.
  • Macrocytic, normochromic anemia: Elevated MCV – characteristic of vitamin B12 or folate deficiency.
  • Normocytic, normochromic anemia: Normal MCV and MCH – often associated with chronic disease or acute blood loss.

How Are the Values Measured?

Erythrocyte indices are automatically calculated by a hematology analyzer from an EDTA blood sample. They are part of the standard complete blood count (CBC) and results are typically available within a few hours.

When to See a Doctor

Abnormal erythrocyte indices do not always cause noticeable symptoms. However, if you experience persistent fatigue, pallor, dizziness, shortness of breath, or rapid heartbeat, you should consult a healthcare professional. Early diagnosis allows for targeted treatment of the underlying cause.
