Estriol measurement is a laboratory diagnostic procedure used to determine the level of the female sex hormone estriol (E3) in blood or urine. Estriol belongs to the group of estrogens and is the weakest of the three main naturally occurring estrogens (alongside estradiol and estrone). During pregnancy, estriol is produced in large quantities by the placenta and the fetal liver, making it an important marker for the well-being of both mother and child.
Estriol measurement is applied in the following clinical situations:
The measurement is typically performed on a blood sample (serum) or a 24-hour urine collection. For prenatal screening, unconjugated estriol (uE3) in maternal blood is most commonly measured, as it reflects fetal development most accurately. The sample is analyzed in the laboratory using immunoassay techniques.
As part of the Triple or Quadruple Test, estriol measurement is generally performed between weeks 15 and 20 of pregnancy. Results are always interpreted in combination with other markers (e.g., AFP, beta-hCG, inhibin A), as well as maternal age and ultrasound findings.
Normal estriol values depend strongly on the timing of measurement and individual context. During pregnancy, estriol levels rise continuously. Deviations from the reference range are typically expressed as a multiple of the median (MoM):
Estriol measurement alone is not a definitive diagnostic test. An abnormal result does not necessarily indicate a medical condition. Results must always be interpreted in the overall context of medical history, other laboratory values, and imaging studies (e.g., ultrasound, or amniocentesis if indicated). Consultation with a specialist in gynecology or prenatal diagnostics is strongly recommended.
