Definition

The facies diaphragmatica pulmonis (Latin for diaphragmatic surface of the lung) refers to the inferior surface of each lung, which rests directly on the diaphragm. Its concave shape mirrors the convex upper surface of the diaphragm. This anatomical surface is a standard term in medical education and is regularly referenced in clinical diagnostics when assessing the lower lung fields.

Anatomical Location and Structure

Each lung has several surfaces named after their neighboring structures. The facies diaphragmatica forms the base of the lung (basis pulmonis) and is bounded superiorly and laterally by the sharp inferior border of the lung (margo inferior pulmonis).

  • Right lung: The diaphragmatic surface of the right lung is broader and positioned slightly higher, as the right dome of the diaphragm is elevated by the liver beneath it.
  • Left lung: The diaphragmatic surface of the left lung is narrower and positioned slightly lower, corresponding to the stomach and spleen lying below the left hemidiaphragm.

Clinical Relevance

The facies diaphragmatica is clinically significant in several conditions and diagnostic procedures:

  • Pleural effusion: Fluid accumulations in the pleural cavity initially collect near the costodiaphragmatic recess, which is located in close proximity to the diaphragmatic surface.
  • Elevated diaphragm: Conditions such as hepatomegaly (enlarged liver) or ascites can push the diaphragm upward, compressing the diaphragmatic surface and reducing lung capacity.
  • Basal atelectasis: Collapse of lung tissue at the base is frequently observed postoperatively or in bedridden patients, directly affecting the diaphragmatic surface area.
  • Imaging: In chest X-rays and CT scans, the relationship between the inferior lung border and the diaphragm is routinely assessed to detect pathological changes.

Topographical Relationships

The facies diaphragmatica is in close topographical relation to the abdominal organs situated below the diaphragm. Diseases of these organs can directly affect the lung through the diaphragm:

  • Right side: liver and right kidney
  • Left side: stomach, spleen, and left kidney

These close anatomical relationships explain why subphrenic processes -- such as subphrenic abscesses -- can lead to reactive pleural effusions or respiratory compromise.
