Fatty liver disease (hepatic steatosis) occurs when excess fat accumulates in liver cells. It is commonly caused by alcohol, poor diet, or metabolic conditions such as obesity and type 2 diabetes.

What is Fatty Liver Disease?

Fatty liver disease, medically known as hepatic steatosis or steatosis hepatis, is one of the most common liver conditions worldwide. It develops when too much fat – primarily triglycerides – builds up inside liver cells (hepatocytes). A liver is considered fatty when more than 5 % of its cells contain fat deposits. In many cases, the condition causes no symptoms in its early stages and is often detected incidentally during an abdominal ultrasound.

Types of Fatty Liver Disease

There are two main forms:

  • Alcoholic liver disease (ALD): Caused by regular or excessive alcohol consumption.
  • Non-alcoholic fatty liver disease (NAFLD/MASLD): Occurs independently of alcohol and is closely associated with obesity, type 2 diabetes, and metabolic syndrome.

Causes

Fatty liver disease has multiple potential causes:

  • Overweight and obesity: Excess body fat and insulin resistance promote fat storage in the liver.
  • Alcohol consumption: Alcohol significantly disrupts fat metabolism in the liver.
  • Type 2 diabetes and insulin resistance: Elevated insulin levels encourage fat accumulation in the liver.
  • Unhealthy diet: High intake of sugar, sugary beverages, fast food, and saturated fats.
  • Physical inactivity: Reduced calorie expenditure contributes to fat accumulation.
  • Medications: Certain drugs (e.g., corticosteroids, tamoxifen) can promote fatty liver.
  • Genetic factors: Certain gene variants (e.g., PNPLA3) increase susceptibility.

Symptoms

Fatty liver disease is often asymptomatic in its early stages. As the condition progresses, the following symptoms may appear:

  • Fatigue and general weakness
  • Dull pressure or mild pain in the upper right abdomen
  • Digestive discomfort and bloating
  • Reduced physical performance
  • In advanced stages: yellowing of the skin and eyes (jaundice), fluid retention (edema), confusion

Diagnosis

Fatty liver disease is diagnosed through several methods:

  • Blood tests: Elevated liver enzymes (ALT, AST, GGT) may indicate liver involvement, although they can be normal in early fatty liver disease.
  • Ultrasound (sonography): A fatty liver appears brighter than normal on ultrasound. This is the most common initial diagnostic tool.
  • Liver elastography (FibroScan): Measures liver stiffness to assess the degree of fat accumulation and fibrosis.
  • MRI of the liver: A highly accurate method for quantifying liver fat content.
  • Liver biopsy: The gold standard for assessing the extent of fat accumulation, inflammation, and fibrosis; however, it is invasive and only used when necessary.

Treatment

There are currently limited specific drug treatments for fatty liver disease. Management focuses primarily on lifestyle changes:

  • Weight loss: Losing 7–10 % of body weight can significantly reduce liver fat.
  • Dietary changes: A Mediterranean diet, reducing sugar and saturated fats, and increasing vegetables, fiber, and unsaturated fatty acids.
  • Physical activity: Regular exercise (at least 150 minutes of moderate aerobic activity per week) improves fat metabolism.
  • Alcohol abstinence: Complete avoidance of alcohol is essential in alcoholic fatty liver disease.
  • Treatment of underlying conditions: Optimal management of diabetes, high blood pressure, and elevated blood lipids.
  • Medications: Newer agents such as resmetirom (approved for MASH) and GLP-1 receptor agonists show promising results in treating non-alcoholic fatty liver disease.

Prognosis and Disease Progression

Fatty liver disease is fully reversible in its early stages. Without treatment, however, it can progress to non-alcoholic steatohepatitis (NASH/MASH) (liver inflammation), then to liver fibrosis (scarring), liver cirrhosis, and in rare cases to hepatocellular carcinoma (liver cancer). Early diagnosis and consistent lifestyle modifications are therefore key to a favorable outcome.
