What is a Fibula Fracture?

A fibula fracture is a break in the fibula, the thinner of the two lower leg bones running along the outer side of the leg, parallel to the tibia (shinbone). The fibula plays a key role in the structure and stability of the ankle joint. Fibula fractures are among the most common bone injuries of the lower extremity and can range from minor stable breaks to complex fractures involving the ankle joint.

Causes

Fibula fractures occur due to a variety of mechanisms:

  • Ankle rolling or twisting (supination trauma): The most frequent cause, especially during sports activities
  • Falls from height or on uneven surfaces
  • Direct impact, such as in motor vehicle accidents
  • Stress fractures: Caused by repetitive loading, common in runners and endurance athletes
  • Osteoporosis: Reduced bone density increases fracture risk, especially in older adults

Symptoms

Typical signs and symptoms of a fibula fracture include:

  • Pain along the outer lower leg or ankle, worsening with weight-bearing
  • Swelling and bruising (hematoma) at the injury site
  • Tenderness on palpation of the fibula
  • Restricted range of motion of the ankle or knee joint
  • Visible deformity in displaced fractures
  • Inability to bear weight in severe cases

Classification

Fibula fractures involving the ankle are commonly classified using the Weber classification:

  • Weber A: Fracture below the syndesmosis (the fibrous joint connecting the tibia and fibula); the syndesmosis remains intact
  • Weber B: Fracture at the level of the syndesmosis; the syndesmosis may be partially injured
  • Weber C: Fracture above the syndesmosis; the syndesmosis is typically disrupted, leading to joint instability

Fractures are also categorized as stable or unstable, isolated or combined (e.g., with medial malleolus involvement), and closed or open.

Diagnosis

The diagnosis of a fibula fracture is established through:

  • Physical examination: Assessment of tenderness, swelling, and joint stability
  • X-ray imaging in two planes: Standard method to visualize and classify the fracture
  • Computed tomography (CT): Used for complex fractures or detailed pre-surgical planning
  • Magnetic resonance imaging (MRI): Helpful for detecting ligament injuries or stress fractures

Treatment

Conservative Treatment

Stable, minimally displaced fractures (particularly Weber A and many Weber B fractures) are often managed non-surgically:

  • Immobilization with a below-knee cast or removable orthosis (ankle brace)
  • Non-weight-bearing or partial weight-bearing with crutches for several weeks
  • Pain management using anti-inflammatory medications (e.g., ibuprofen, paracetamol)
  • Elevation of the affected leg to reduce swelling
  • Ice application in the acute phase

Surgical Treatment

Unstable, displaced, or complex fractures often require surgery:

  • Open reduction and internal fixation (ORIF): Bone stabilization using screws, plates, or intramedullary nails
  • Syndesmotic screw fixation when the syndesmosis is disrupted, to restore ankle joint stability

Rehabilitation

Following immobilization, physiotherapy is essential to restore strength, range of motion, and proprioception. Full recovery typically takes 6 to 12 weeks, and longer for complex or surgically treated fractures.

Complications

Potential complications of a fibula fracture include:

  • Malunion or delayed bone healing
  • Ankle instability from unrecognized ligament injuries
  • Post-traumatic osteoarthritis of the ankle joint
  • Deep vein thrombosis (DVT) due to immobilization
  • Wound healing problems following surgery
