Definition and Anatomy

The foramen costotransversarium (also referred to as the foramen costotransversale) is an anatomical opening formed between the neck of a rib (collum costae) and the transverse process (processus transversus) of the corresponding thoracic vertebra. This space arises from the spatial arrangement of bony structures in the thoracic spine and forms part of the functional unit of the costotransverse joints.

The foramen is present bilaterally at each of the twelve thoracic vertebrae (T1–T12) and is an integral component of the complex rib-vertebra joint apparatus that ensures the stability of the thoracic cage and facilitates breathing movements.

Contents and Function

The costotransverse ligament (ligamentum costotransversarium) passes through the foramen costotransversarium. This ligament, which includes the superior costotransverse ligament and the lateral costotransverse ligament, connects the rib neck to the transverse process and secures the costotransverse joint (articulatio costotransversaria), which coordinates rib movement during respiration.

  • Stabilization: The costotransverse ligament stabilizes the joint between the rib and the transverse process, limiting excessive movement.
  • Respiratory function: The mobility of this joint allows the ribs to rise and fall during inhalation and exhalation (pump-handle and bucket-handle movements).
  • Neural structures: The dorsal rami of the spinal nerves, which supply the back muscles and the skin of the back, run in close proximity to the foramen.

The foramen costotransversarium is significant in various clinical contexts:

Pain Syndromes and Nerve Blocks

Inflammatory or degenerative changes in the costotransverse joints can cause localized back pain or radiating pain along the ribs (costotransverse pain syndrome). In interventional pain management, costotransverse joint blocks are performed by injecting a local anesthetic into the joint region. The foramen costotransversarium serves as an important anatomical landmark during this procedure.

Intercostal Nerve Block

When performing an intercostal nerve block for pain management (e.g., following rib fractures or in cases of herpes zoster), precise knowledge of the surrounding structures, including the foramen costotransversarium, is essential to avoid complications such as pneumothorax.

Radiological Diagnostics

In imaging diagnostics, particularly computed tomography (CT) and magnetic resonance imaging (MRI) of the thoracic spine, the foramen costotransversarium is an important anatomical reference point. Pathological changes such as narrowing of the foramen due to osteophytes (bony outgrowths), fractures, or inflammatory processes can be diagnostically relevant.

Tuberculous Spondylitis (Pott Disease)

In tuberculous spondylitis (Pott disease), a so-called paravertebral abscess can spread along the spine. The anatomical region around the foramen costotransversarium may serve as a pathway for this spread, which must be considered during surgical planning.

Development and Variants

The foramen costotransversarium develops during embryogenesis through the differentiation of vertebral arches and rib segments. Anatomical variants in the size and shape of the foramen are possible and may influence the clinical approach during interventional procedures. At the lower thoracic vertebrae (T11, T12), which bear the so-called floating ribs, the costotransverse joint is less developed or absent, resulting in anatomical variation of the foramen at these levels.
