Things worth knowing about "Frailty"

Frailty is a geriatric syndrome characterized by reduced physical reserves, increased vulnerability, and diminished resilience to stressors, most commonly affecting older adults.

What is Frailty?

Frailty is a recognized geriatric syndrome in which a person's physical reserves and ability to withstand external stressors are significantly reduced. It predominantly affects older adults and is associated with a substantially increased risk of falls, hospitalization, long-term care placement, and premature death. Frailty is distinct from normal aging and represents a clinically significant condition that requires targeted medical attention.

Causes and Risk Factors

Frailty results from the interplay of biological, psychological, and social factors:

  • Muscle loss (sarcopenia): Progressive loss of muscle mass and strength with aging is a central driver of frailty.
  • Chronic diseases: Conditions such as heart failure, diabetes mellitus, chronic kidney disease, and cancer significantly contribute to the development of frailty.
  • Malnutrition: Inadequate intake of calories, protein, and micronutrients weakens the body over time.
  • Physical inactivity: Lack of regular exercise accelerates muscle loss and functional decline.
  • Social isolation and depression: Psychological burden and lack of social engagement reinforce physical deterioration.
  • Polypharmacy: The concurrent use of multiple medications can cause side effects that contribute to frailty.

Symptoms

The most widely used definition describes frailty through five key criteria:

  • Unintentional weight loss (more than 4.5 kg in the past year)
  • Exhaustion or persistent fatigue
  • Reduced muscle strength (e.g., weak grip strength)
  • Slowed walking speed
  • Low levels of physical activity

When three or more of these criteria are present, a person is classified as frail. One or two criteria indicate a state of pre-frailty, which represents an important opportunity for early intervention.

Diagnosis

Frailty is assessed through comprehensive geriatric evaluation. Common tools include:

  • Fried Frailty Phenotype: Evaluates the five criteria listed above to classify patients as robust, pre-frail, or frail.
  • Clinical Frailty Scale (CFS): A nine-point scale assessing overall functional status and independence.
  • Frailty Index: Calculates a numerical score based on the cumulative number of health deficits present.
  • Gait speed and grip strength tests: Simple, quick assessments commonly used in clinical practice.

Treatment and Prevention

Frailty, especially when identified early, is a treatable and potentially reversible condition. Key interventions include:

Physical Exercise

Regular resistance and balance training is the most effective intervention for frailty. It improves muscle strength and gait stability and reduces the risk of falls. At least two to three training sessions per week are recommended.

Nutritional Optimization

Adequate protein intake (approximately 1.2 to 1.5 g per kg of body weight per day), along with sufficient vitamins (particularly vitamin D) and minerals, supports muscle preservation and overall health.

Management of Underlying Conditions

Consistent treatment of chronic conditions such as heart failure, diabetes mellitus, or anemia can positively influence the course of frailty.

Medication Review

A thorough review of all medications by a physician helps identify and reduce unnecessary or harmful drug interactions contributing to frailty.

Psychosocial Support

Social engagement, psychological counseling, and the treatment of depression are essential components of a holistic frailty management plan.

Prognosis

Without intervention, frailty typically progresses and significantly increases the risk of dependence on care and mortality. However, with early identification and consistent treatment, the progression of frailty can be slowed or even reversed. Regular geriatric health screenings play a key role in achieving this outcome.
