What is Fundal Height?
Fundal height refers to the distance from the upper edge of the pubic bone (symphysis) to the top of the uterus, known as the fundus uteri. As the pregnancy progresses, the uterus grows upward toward the navel and rib cage. Measuring this distance at regular prenatal checkups provides important information about fetal growth and overall pregnancy development.
How is Fundal Height Measured?
Fundal height is measured by a midwife or obstetrician using a soft measuring tape. The tape is placed from the top of the pubic symphysis to the highest point of the uterus. The result is recorded in centimeters in the prenatal record. This simple, non-invasive measurement is a routine part of antenatal care.
Expected Measurements by Gestational Week
- 12th week of pregnancy: Uterine fundus just above the pubic symphysis
- 20th week: Fundus at the level of the navel
- 28th week: Fundus approximately 3–4 finger-widths above the navel
- 36th week: Fundus reaches the lower rib margin
- 40th week (due date): Fundus slightly lower as the baby descends into the pelvis (lightening)
Clinical Significance
Fundal height is a simple and cost-effective screening tool in prenatal care. A significant deviation from the expected measurement for a given gestational age may indicate various clinically relevant conditions:
- Higher than expected fundal height: may suggest a multiple pregnancy, polyhydramnios (excess amniotic fluid), or fetal macrosomia (large baby)
- Lower than expected fundal height: may indicate intrauterine growth restriction (IUGR) or oligohydramnios (low amniotic fluid)
In such cases, an ultrasound examination is typically ordered to further evaluate the findings.
Symphysis-Fundal Distance
In modern prenatal care, the measurement is often referred to as the symphysis-fundal distance (SFD). As a general rule of thumb, the measurement in centimeters should roughly correspond to the gestational age in weeks (e.g., approximately 28 cm at 28 weeks). A deviation of more than 2–3 cm may warrant further investigation by a healthcare professional.
