What is Gemfibrozil?
Gemfibrozil is a lipid-lowering drug belonging to the class of fibrates. It is primarily used to reduce elevated triglyceride levels in the blood and to raise HDL cholesterol (the so-called good cholesterol). The drug has been in clinical use since the 1980s and is one of the established agents in the treatment of dyslipidaemia (disorders of fat metabolism).
Indications
Gemfibrozil is mainly used in the following conditions:
- Hypertriglyceridaemia: markedly elevated triglyceride levels in the blood, which can increase the risk of pancreatitis
- Mixed hyperlipidaemia: simultaneously elevated triglyceride and cholesterol levels
- Primary prevention in patients with low HDL cholesterol and elevated cardiovascular risk, when statin therapy is not possible or insufficiently effective
Mechanism of Action
Gemfibrozil acts by activating PPAR-alpha (peroxisome proliferator-activated receptor alpha), a nuclear receptor found in liver cells and other tissues. This activation influences several metabolic processes:
- Enhanced lipolysis: activation of lipoprotein lipase (LPL) reduces the release of free fatty acids from adipose tissue into the bloodstream
- Reduced triglyceride synthesis in the liver
- Increased production of apolipoprotein A-I and A-II, the main components of HDL cholesterol
- Accelerated clearance of VLDL particles (very-low-density lipoprotein) from the circulation
The overall result is a significant reduction in triglycerides (by 20–50%) and a moderate increase in HDL cholesterol (by 10–20%). The effect on LDL cholesterol is variable and often less pronounced.
Dosage and Administration
Gemfibrozil is typically taken orally at a dose of 600 mg twice daily, 30 minutes before breakfast and the evening meal. The exact dosage should always follow medical advice. Regular monitoring of blood lipid levels is necessary during treatment.
Side Effects
Like all medications, gemfibrozil can cause side effects. Common and important adverse effects include:
- Gastrointestinal complaints: abdominal pain, nausea, vomiting, diarrhoea, or constipation
- Myopathy: muscle pain or muscle weakness, especially when taken together with statins (increased risk of rhabdomyolysis)
- Elevated liver enzymes: increase in transaminase levels
- Gallstones: gemfibrozil may promote gallstone formation
- Headache and dizziness
- Rarely: blood count changes (leucopenia, thrombocytopenia)
Drug Interactions
Special caution is required with the following combinations:
- Statins (e.g. simvastatin, lovastatin): greatly increased risk of myopathy and rhabdomyolysis – this combination should generally be avoided
- Oral anticoagulants (e.g. warfarin, phenprocoumon): potentiation of anticoagulant effect possible; regular INR monitoring is required
- Repaglinide: markedly elevated plasma levels of the antidiabetic drug; combination is contraindicated
- Colchicine: increased risk of myopathy
Contraindications
Gemfibrozil must not be taken in cases of:
- Severe hepatic or renal impairment
- Known gallbladder disease or gallstones
- Known hypersensitivity to gemfibrozil or related fibrates
- Pregnancy and breastfeeding (due to insufficient safety data)
