Things worth knowing about "GLP-1 agonists"

GLP-1 agonists are medications used to treat type 2 diabetes and obesity. They mimic the natural hormone GLP-1 to lower blood sugar and reduce appetite.

What are GLP-1 Agonists?

GLP-1 agonists (also known as GLP-1 receptor agonists) are a class of medications that mimic the action of the naturally occurring hormone glucagon-like peptide-1 (GLP-1). This hormone is released by the small intestine after eating and plays a key role in regulating blood sugar levels and feelings of fullness. GLP-1 agonists are primarily used to treat type 2 diabetes mellitus and, increasingly, obesity (severe overweight).

Mechanism of Action

GLP-1 agonists bind to GLP-1 receptors throughout the body, producing effects similar to those of the natural hormone. The key effects include:

  • Blood sugar lowering: They stimulate the pancreas to release insulin, but only when blood sugar is elevated. This means the risk of hypoglycemia (dangerously low blood sugar) is lower compared to some other diabetes medications.
  • Glucagon suppression: They reduce the release of glucagon, a hormone that raises blood sugar levels.
  • Slowed gastric emptying: Food stays in the stomach longer, leading to a prolonged feeling of fullness.
  • Brain effects: They act on the hunger-regulating centers in the brain, helping to reduce appetite.

Indications

GLP-1 agonists are used in the following medical situations:

  • Type 2 diabetes mellitus: As monotherapy or in combination with other antidiabetic agents such as metformin, when blood sugar control is insufficient.
  • Obesity: Certain GLP-1 agonists such as semaglutide are approved at higher doses specifically for weight management.
  • Cardiovascular protection: Clinical studies have demonstrated that some GLP-1 agonists reduce the risk of heart attack and stroke in high-risk patients.

Available Medications

GLP-1 agonists approved in many countries include:

  • Semaglutide (e.g., Ozempic®, Wegovy®)
  • Liraglutide (e.g., Victoza®, Saxenda®)
  • Dulaglutide (e.g., Trulicity®)
  • Exenatide (e.g., Byetta®, Bydureon®)
  • Tirzepatide (e.g., Mounjaro®; a dual GIP/GLP-1 receptor agonist)

Dosage and Administration

Depending on the specific medication, GLP-1 agonists are available as subcutaneous injections (injected under the skin) or as oral tablets (e.g., oral semaglutide). The frequency of administration varies:

  • Daily: e.g., liraglutide, exenatide (twice daily)
  • Weekly: e.g., semaglutide, dulaglutide, tirzepatide

The dose is typically started low and gradually increased to minimize side effects. These medications are always prescribed and supervised by a physician.

Side Effects

GLP-1 agonists are generally well tolerated, but side effects may occur, especially at the start of treatment:

  • Common: Nausea, vomiting, diarrhea, constipation, reduced appetite
  • Occasional: Abdominal pain, heartburn, fatigue
  • Rare but serious: Pancreatitis (inflammation of the pancreas), gallbladder problems, kidney function impairment
  • Injection site reactions: Redness, itching, or mild swelling

Individuals with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 (MEN-2) should not use GLP-1 agonists.

Advantages Over Other Antidiabetic Medications

Compared to some other diabetes treatments, GLP-1 agonists offer several benefits:

  • Low risk of hypoglycemia (low blood sugar)
  • Support for weight loss (unlike some other antidiabetics that may cause weight gain)
  • Proven cardiovascular protective effects with certain agents
  • Potential protective effects on kidney function
