What is Guselkumab?

Guselkumab is a monoclonal antibody belonging to the class of biologics. It is marketed under the brand name Tremfya and is approved for the treatment of adults with moderate to severe plaque psoriasis and psoriatic arthritis. The medication was developed and is marketed by Janssen Pharmaceuticals.

Mechanism of Action

Guselkumab specifically binds to the p19 subunit of interleukin-23 (IL-23), a signaling molecule (cytokine) that plays a central role in inflammatory processes. By selectively blocking IL-23, guselkumab inhibits the overactivation of the immune system that drives the inflammation and tissue damage seen in psoriasis and psoriatic arthritis.

  • Inhibition of the downstream inflammatory cascade
  • Reduction of skin and joint inflammation

Indications

Guselkumab is approved for:

  • Plaque psoriasis: moderate to severe forms in adults when other therapies (e.g., phototherapy, methotrexate) are not sufficiently effective or appropriate
  • Psoriatic arthritis: active disease in adults, either alone or in combination with methotrexate

Dosage and Administration

Guselkumab is administered as a subcutaneous injection (under the skin). The standard dosing schedule for psoriasis is:

  • 100 mg at the start of therapy (Week 0)
  • 100 mg at Week 4
  • Then 100 mg every 8 weeks as maintenance therapy

The injection can be administered by a healthcare professional or, after appropriate training, by the patient themselves. The product is available as a prefilled syringe or autoinjector pen.

Side Effects

Like all biologics, guselkumab can cause side effects. Common side effects include:

  • Injection site reactions (redness, swelling, pain)
  • Upper respiratory tract infections (e.g., common cold, nasopharyngitis)
  • Headache
  • Joint pain (arthralgia)
  • Diarrhea

Less commonly, serious infections or hypersensitivity reactions may occur. Before starting therapy, patients should be screened for tuberculosis and other chronic infections.

Contraindications and Precautions

Guselkumab must not be used in patients with:

  • Known hypersensitivity to the active substance or any excipients
  • Active, clinically significant infections

Special caution is required in patients with pre-existing conditions such as chronic infections, liver disease, or a history of malignancy. Use during pregnancy and breastfeeding should be discussed with the treating physician, as available data are limited.

Clinical Efficacy

In clinical trials (e.g., VOYAGE-1, VOYAGE-2, NAVIGATE), guselkumab demonstrated significant superiority over placebo and, in some studies, over the comparator adalimumab in terms of skin improvement as measured by the PASI score. Many patients achieved near-complete or complete skin clearance.
