What is Half-Life?

The half-life (abbreviated as t½ or HWZ in German) is a fundamental medical and pharmacological concept that describes the time required for the concentration of a substance, drug, or radioactive isotope to decrease to half of its initial value in the body or a given system. It is a measure of how quickly a substance is metabolized, excreted, or undergoes radioactive decay.

Types of Half-Life

Biological Half-Life

The biological half-life refers to the time the body needs to eliminate half of an ingested or administered substance through metabolic processes and excretion (e.g., via the kidneys, liver, or lungs). It is especially relevant in pharmacology for determining the correct dosing intervals of medications.

Pharmacokinetic Half-Life

In pharmacology, the half-life is used to estimate when a medication is fully eliminated from the body. After approximately 4 to 5 half-lives, a drug is considered to be completely cleared. It directly influences dosing frequency: drugs with a short half-life must be taken more frequently than those with a long half-life.

Physical (Radioactive) Half-Life

The physical half-life is a concept from nuclear physics and nuclear medicine. It describes the time it takes for half of the atoms in a radioactive isotope to decay. In nuclear medicine, radioactive substances with intentionally short half-lives are used to minimize radiation exposure for the patient.

Effective Half-Life

The effective half-life combines the biological and physical half-lives. It describes how quickly a radioactive substance disappears from the body overall, taking into account both radioactive decay and biological excretion.

Importance in Medicine

The half-life is an indispensable concept across several medical disciplines:

  • Dosing schedules: Physicians and pharmacists use the half-life to determine appropriate dosing intervals and maintain a stable drug concentration in the blood.
  • Onset and offset of therapy: It helps predict when a drug reaches its full effect and when it is completely eliminated from the body.
  • Drug interactions: Substances with long half-lives remain in the body longer and may interact with other medications for extended periods.
  • Nuclear medicine: The half-life is used in diagnostic and therapeutic procedures to minimize patient radiation exposure.

Examples of Half-Lives of Common Substances

  • Aspirin (acetylsalicylic acid): approx. 15–20 minutes
  • Ibuprofen: approx. 2 hours
  • Diazepam (sedative): approx. 20–100 hours
  • Levothyroxine (thyroid hormone): approx. 7 days
  • Technetium-99m (nuclear medicine): approx. 6 hours (physical)

Factors Influencing Half-Life

The half-life of a substance can be affected by several factors:

  • Age: In elderly individuals or newborns, drug metabolism may be slower, prolonging the half-life.
  • Liver and kidney function: Impaired organ function slows down breakdown and excretion, leading to a longer half-life.
  • Genetic factors: Individual differences in metabolic enzymes (e.g., CYP450 enzymes) influence the rate of drug elimination.
  • Drug interactions: Other medications can accelerate or inhibit the metabolism of a substance, altering its effective half-life.
