What Is a Herniated Disc?

A herniated disc (also called a slipped or prolapsed disc) occurs when the soft inner core of an intervertebral disc pushes through the outer fibrous ring and presses on nearby nerves or the spinal cord. This can cause significant pain, numbness, and limited mobility. Treatment depends on the severity of symptoms and the location of the affected disc in the spine.

Conservative Treatment

The majority of herniated disc cases can be managed successfully without surgery. Conservative therapy is generally the first line of treatment and involves several approaches.

Pain Management

Common pain relievers such as ibuprofen or diclofenac are used to reduce discomfort. For more severe cases, muscle relaxants or short-term mild opioids may be prescribed. Corticosteroid injections near the affected nerve (epidural steroid injections) can help reduce inflammation and provide targeted relief.

Physiotherapy and Exercise

Targeted physiotherapy is a cornerstone of herniated disc treatment. Specific exercises strengthen the back and core muscles, reduce pressure on the spine, and improve flexibility. While rest may be appropriate in the acute phase, gentle movement is encouraged to promote healing and prevent muscle weakening.

Additional Conservative Approaches

  • Heat and cold therapy to relax muscles and ease pain
  • Manual therapy performed by trained physiotherapists or osteopaths
  • Acupuncture as a complementary method for pain reduction
  • Electrotherapy (e.g., TENS units) for pain relief
  • Psychological support for patients with chronic pain

Surgical Treatment

Surgery is recommended when conservative treatment has not provided sufficient relief after approximately 6 to 12 weeks, when neurological deficits such as weakness or severe numbness develop, or when bladder or bowel function is affected (cauda equina syndrome – a medical emergency).

Microdiscectomy

Microdiscectomy is the most common surgical procedure for a herniated disc. Under microscopic guidance, the protruding portion of the disc is removed to relieve pressure on the nerve. This minimally invasive procedure typically involves a short recovery time.

Endoscopic Procedures

Modern endoscopic techniques allow even less invasive removal of the herniated tissue through very small incisions. These procedures are performed at specialized centers and have high success rates.

Disc Replacement and Spinal Fusion

In rare cases, an artificial disc replacement or spinal fusion (spondylodesis) may be performed when the disc is severely damaged and less invasive options are insufficient to restore function.

Rehabilitation and Prevention

Following treatment, targeted rehabilitation is essential to prevent recurrence. This includes regular back-strengthening exercises, learning spine-friendly movement patterns, and maintaining good posture. Low-impact sports such as swimming, cycling, and yoga are particularly well-suited for long-term prevention.
