Histamine is a naturally occurring chemical messenger involved in numerous physiological processes, but when it accumulates in excessive amounts, it can trigger a wide range of symptoms.

In most cases of histamine intolerance, there is a deficiency or reduced activity of the enzyme diamine oxidase (DAO), which is responsible for breaking down histamine in the intestine. As a result, dietary histamine enters the bloodstream and causes symptoms. Common complaints include:

  • Headaches and migraines
  • Skin reactions such as flushing, itching, or hives
  • Digestive issues such as bloating, diarrhea, or abdominal cramps
  • Runny nose and nasal congestion
  • Heart palpitations and blood pressure fluctuations

Core Principles of the Histamine Elimination Diet

The histamine elimination diet is based on three key principles: avoiding histamine-rich foods, avoiding histamine liberators (substances that trigger the release of the body's own histamine), and avoiding DAO inhibitors (substances that block histamine degradation).

Histamine-Rich Foods (to avoid)

  • Aged cheeses (e.g., Camembert, Gouda, Parmesan)
  • Fermented products (e.g., sauerkraut, yogurt, kefir, soy sauce)
  • Alcoholic beverages, especially red wine and beer
  • Smoked and cured meat and sausage products
  • Canned fish, tuna, mackerel, sardines
  • Vinegar and vinegar-containing products
  • Yeast and yeast extracts

Histamine Liberators (to avoid)

  • Strawberries, tomatoes, citrus fruits
  • Chocolate and cocoa
  • Nuts (especially walnuts and cashews)
  • Seafood and shellfish
  • Pork
  • Spinach and avocado

DAO Inhibitors (to avoid)

  • Alcohol (directly inhibits DAO activity)
  • Black tea, green tea, energy drinks
  • Certain medications (e.g., some antibiotics, pain relievers, antidepressants – always consult a physician)

Well-Tolerated Foods on the Histamine Elimination Diet

In addition to avoiding problematic foods, there is a wide variety of well-tolerated alternatives that can be freely consumed or tested individually:

  • Fresh meat (not cured or smoked)
  • Fresh fish (processed immediately, not stored)
  • Most vegetables (except tomatoes, spinach, avocado)
  • Potatoes, rice, corn, millet
  • Fresh herbs (except chives and stinging nettle)
  • Most fruits (except citrus fruits, strawberries, bananas, pineapple)
  • Fresh milk and butter (in moderation, test individually)
  • Plant-based oils such as olive oil or coconut oil

Practical Implementation

The histamine elimination diet is typically carried out in two phases. During the elimination phase (usually 4–8 weeks), all histamine-rich and histamine-releasing foods are strictly avoided to reduce the body's histamine load and monitor symptom improvement. This is followed by a reintroduction phase, in which individual foods are gradually reintroduced to determine personal tolerance thresholds. Since histamine intolerance varies greatly between individuals, the diet should ideally be conducted under the guidance of a nutritional medicine specialist.

Complementary Measures

In addition to dietary changes, the following measures may provide additional support:

  • DAO enzyme supplements: Available as dietary supplements, these can be taken before histamine-rich meals to support histamine breakdown.
  • Vitamin B6, Vitamin C, and copper: These micronutrients act as co-factors of DAO and may promote histamine degradation when adequately supplied.
  • Stress reduction: Chronic stress can impair DAO activity and worsen histamine-related symptoms.
