What is the Hueter Triangle?

The Hueter triangle is an important anatomical landmark used in the clinical examination of the elbow joint. It is named after the German surgeon Carl Hueter (1838–1882). The triangle describes the geometric relationship between three bony prominences of the elbow: the lateral epicondyle, the medial epicondyle, and the olecranon.

Anatomical Basis

When the elbow is fully extended, all three bony points lie on a straight line, known as the Hueter line. When the elbow is flexed to 90 degrees, these three points form an isosceles triangle -- the Hueter triangle. This geometric arrangement is consistent and reproducible under normal anatomical conditions.

  • Lateral epicondyle: The bony prominence on the outer side of the humerus (upper arm bone)
  • Medial epicondyle: The bony prominence on the inner side of the humerus
  • Olecranon: The bony projection of the ulna that forms the tip of the elbow

Clinical Significance

The Hueter triangle is a valuable diagnostic tool, allowing the examiner to quickly assess possible injuries or deformities of the elbow without imaging studies.

Normal Findings

In a healthy, uninjured elbow, the triangle formed at 90 degrees of flexion is isosceles and symmetrical, with equal distances between the three bony landmarks.

Pathological Changes

Distortion of the normal triangular shape indicates a possible injury or disorder:

  • Elbow dislocation: The olecranon is displaced, and the triangle is distorted or no longer recognizable.
  • Epicondyle fractures: An avulsion or fracture of one epicondyle disrupts the symmetrical triangular shape.
  • Supracondylar humerus fracture: Common in children; the triangle is often preserved in this type of fracture, which is an important distinguishing feature from elbow dislocation.

Diagnosis and Examination

Assessment of the Hueter triangle is performed by palpation of the three bony landmarks with the elbow flexed at 90 degrees. The examiner identifies the medial epicondyle, lateral epicondyle, and olecranon and evaluates whether the typical triangular arrangement is intact.

This straightforward examination technique is particularly useful in the presence of swelling, as the bony landmarks often remain palpable even with significant soft tissue edema. It complements imaging studies such as X-rays and helps confirm or establish clinical diagnoses.

Distinction from the Hueter Line

Closely related to the Hueter triangle is the Hueter line: with the elbow in full extension, the medial epicondyle, lateral epicondyle, and olecranon are aligned on a straight line. Any deviation from this alignment in the extended position also suggests a pathological change. The Hueter triangle (elbow flexed) and the Hueter line (elbow extended) are used together as clinical diagnostic signs during examination of the elbow.
