Human Milk Oligosaccharides (HMOs) are a highly diverse group of complex carbohydrates found naturally in human breast milk. At concentrations of 5 to 20 grams per liter, they are the third most abundant solid component of breast milk, after lactose and fats. More than 200 distinct HMO structures have been identified to date. The most prevalent include 2'-Fucosyllactose (2'-FL) and Lacto-N-neotetraose (LNnT).
HMOs perform a wide range of important biological functions in the infant body:
HMOs are found almost exclusively in human breast milk. Their composition and concentration vary individually between mothers and depend on the stage of lactation (colostrum, transitional milk, mature milk) as well as the mother's blood group characteristics (secretor status). Cow's milk contains structurally similar compounds called bovine Milk Oligosaccharides (bMOs), but in much lower variety and concentration.
Since breastfed infants demonstrably enjoy health advantages over formula-fed infants, there is considerable interest in incorporating HMOs into infant formula. Today, select HMOs — primarily 2'-FL and LNnT — are produced biotechnologically and added to commercial infant formulas. Clinical studies show that these enriched formulas are well tolerated and may positively influence certain immune markers.
Research into HMOs has grown substantially over the past two decades. Clinical studies and animal experiments support prebiotic, anti-infective, and immunomodulatory effects. Particularly well-studied is the role of HMOs in reducing the risk of necrotizing enterocolitis (NEC) in preterm infants and in promoting a healthy gut microbiome. Further research is investigating potential effects on allergies, autoimmune conditions, and cognitive development.
HMOs naturally present in breast milk are considered very safe. Biotechnologically produced HMOs added to infant formulas have also been classified as safe by regulatory authorities including the European Food Safety Authority (EFSA) and the U.S. Food and Drug Administration (FDA). Adverse effects are rare and typically mild.
