What is Hydroxylapatite?

Hydroxylapatite (chemical formula: Ca₁₀(PO₄)₆(OH)₂) is a naturally occurring mineral belonging to the calcium phosphate group. It is the primary inorganic component of human bones and tooth enamel, providing these tissues with their characteristic hardness and structural stability. Approximately 60–70% of the dry weight of bone consists of hydroxylapatite crystals embedded within a network of collagen fibres.

Biological Significance

Within the human body, hydroxylapatite fulfils several essential roles:

  • Structural function: It forms the mineral matrix of bones and teeth, conferring mechanical strength and rigidity.
  • Calcium and phosphate reservoir: Bone serves as a storage site for calcium and phosphate ions, which are released as needed to maintain electrolyte balance.
  • Cell interaction: Hydroxylapatite surfaces promote the attachment and differentiation of bone-forming cells (osteoblasts) and facilitate the binding of proteins and growth factors.

Medical and Dental Applications

Bone Substitution and Implantology

Synthetically produced hydroxylapatite is used in orthopaedics, trauma surgery, and oral and maxillofacial surgery as a bone substitute material. It is applied to fill bone defects resulting from fractures, tumour removal, or dental procedures. Due to its excellent biocompatibility, it is well tolerated by the body and promotes the ingrowth of natural bone (osseointegration).

Implant Coating

Metallic implants -- for example those made of titanium -- are frequently coated with a thin layer of hydroxylapatite to enhance fixation within the bone and accelerate healing. This coating mimics the natural bone surface and facilitates the adhesion of bone cells.

Dentistry and Oral Hygiene

In dentistry, hydroxylapatite is used to remineralise tooth enamel and prevent dental caries. Toothpastes containing nano-hydroxylapatite can fill micro-cracks in enamel and form a protective layer on the tooth surface. Studies indicate that nano-hydroxylapatite demonstrates comparable efficacy to fluoride in enamel remineralisation.

Drug Delivery

Thanks to its porous structure and excellent biocompatibility, hydroxylapatite is also being investigated as a carrier material for the controlled release of drugs, antibiotics, and growth factors in regenerative medicine.

Production and Forms

Synthetic hydroxylapatite can be manufactured in various forms:

  • Granular hydroxylapatite: Granules or particles for filling bone defects
  • Porous blocks: For bridging larger bone defects
  • Nano-hydroxylapatite: Ultrafine particles used in toothpastes and coatings
  • Resorbable variants: Gradually degraded by the body and replaced by natural bone

Safety and Biocompatibility

Hydroxylapatite is considered highly biocompatible, meaning it is well tolerated and not recognised by the immune system as a foreign body. It is neither toxic nor allergenic, and has therefore been safely used in medicine for decades. Synthetic hydroxylapatite is rigorously tested for purity and sterility before being incorporated into medical products.
