What is Hypoallergenic Baby Food?

Hypoallergenic baby food, commonly referred to as HA formula, is a specially developed infant formula designed for babies at an elevated risk of developing food allergies. It is primarily used when breastfeeding is not possible and there is a family history of allergic conditions. The term “hypoallergenic” indicates that the product is formulated to provoke fewer allergic reactions than standard infant formula.

Composition and Mechanism of Action

The defining feature of hypoallergenic baby food is the use of hydrolyzed protein. Cow's milk proteins – the most common trigger of food allergies in infancy – are broken down enzymatically or thermally into smaller fragments (peptides). The infant's immune system is less likely to recognize these fragments as allergens and therefore less likely to mount an allergic response.

Partially hydrolyzed formula (pHA): Proteins are partially broken down. This type is used for allergy prevention in infants with a family history of allergies.
Extensively hydrolyzed formula (eHA): Proteins are broken down to a greater degree. This type is used therapeutically when a cow's milk protein allergy has already been diagnosed.
Amino acid-based formula: Contains individual amino acids instead of peptides and is used in cases of severe allergic reactions.

When is Hypoallergenic Baby Food Recommended?

The use of HA formula is considered in the following situations:

When breast milk is not available and a parent or sibling has a known allergy, asthma, or atopic dermatitis.
For allergy prevention in infants with a documented family history of allergic disease.
When a cow's milk protein allergy or intolerance is suspected or confirmed.

Important: In cases of a confirmed cow's milk protein allergy, partially hydrolyzed formula is generally not sufficient. Extensively hydrolyzed or amino acid-based formula is required in such cases.

Safety and Scientific Evidence

The European Food Safety Authority (EFSA) and the World Health Organization (WHO) emphasize that exclusive breastfeeding for at least six months remains the best form of nutrition for infants. HA formula is considered a safe alternative when breastfeeding is not possible. However, the scientific evidence regarding allergy prevention through partially hydrolyzed formula is mixed: while some studies indicate a protective effect, more recent systematic reviews have shown less conclusive results.

Practical Guidelines for Use

Hypoallergenic baby food is prepared and administered in the same way as standard infant formula. Parents should keep the following points in mind:

HA formula is not a substitute for breastfeeding and should only be used after consulting a pediatrician.
The transition from standard to HA formula should be medically supervised.
HA formula often has a slightly bitter taste that some infants may initially refuse. Gradual introduction can help with acceptance.
The cost is generally higher than that of standard formula. Reimbursement by health insurance may be available with a confirmed diagnosis.

Introduction of Complementary Foods

From the 4th to 6th month of life, infants receiving HA formula can begin the introduction of complementary foods. Current guidelines recommend not delaying the introduction of allergenic foods (e.g., peanuts, eggs, fish), as early exposure may promote tolerance. This process should also be guided by a healthcare professional.
