What is Hypovitaminosis?

Hypovitaminosis is a medical condition in which the body does not receive or maintain adequate levels of one or more essential vitamins. Vitamins are vital organic compounds required for numerous metabolic processes that the body cannot produce on its own in sufficient quantities. Unlike avitaminosis – which describes a complete absence of a vitamin – hypovitaminosis refers to a partial but clinically significant deficiency that can affect overall health and well-being.

Causes

Hypovitaminosis can result from a variety of contributing factors:

  • Inadequate dietary intake: A one-sided or nutrient-poor diet, fasting, restrictive dieting, or certain dietary patterns such as a vegan diet without supplementation can lead to insufficient vitamin consumption.
  • Impaired absorption: Gastrointestinal disorders such as Crohn disease, celiac disease, or gastric bypass surgery can reduce the absorption of vitamins from food.
  • Increased requirements: Certain life stages such as pregnancy, breastfeeding, periods of growth, or intense physical activity significantly raise the body demand for vitamins.
  • Reduced endogenous synthesis: Vitamin D, for example, is produced in the skin through sunlight exposure. Insufficient sun exposure is therefore a common cause of deficiency.
  • Drug interactions: Medications such as proton pump inhibitors, metformin, or antiepileptics can interfere with the absorption or metabolism of specific vitamins.
  • Chronic illness and alcohol use: Liver or kidney disease and chronic alcohol consumption impair the storage and activation of multiple vitamins.

Symptoms

The symptoms of hypovitaminosis vary considerably depending on which vitamin is affected. Common general signs include:

  • Persistent fatigue and exhaustion
  • Difficulties with concentration and memory
  • Weakened immune system and increased susceptibility to infections
  • Changes in skin, hair, and nails
  • Muscle and bone pain

Specific deficiencies are associated with characteristic clinical presentations:

  • Vitamin D deficiency: Bone softening (osteomalacia), muscle weakness, increased risk of osteoporosis
  • Vitamin B12 deficiency: Pernicious anemia, numbness, neurological disorders
  • Vitamin C deficiency: Scurvy with bleeding gums, impaired wound healing, fatigue
  • Vitamin A deficiency: Night blindness, dry skin, weakened immune function
  • Folate deficiency (Vitamin B9): Megaloblastic anemia, neural tube defects in pregnancy

Diagnosis

Diagnosis of hypovitaminosis typically involves a combination of:

  • Medical history: Assessment of dietary habits, pre-existing conditions, medications, and risk factors
  • Physical examination: Evaluation of clinical signs associated with vitamin deficiency
  • Laboratory testing: Measurement of vitamin levels in blood serum or plasma, which is the primary diagnostic method

For some vitamins, additional functional tests are available that provide a more accurate picture of the body overall vitamin status.

Treatment

Treatment is tailored to the specific vitamin involved, the severity of the deficiency, and the underlying cause:

  • Dietary optimization: Adjusting the diet to include vitamin-rich foods – for example, more fruits and vegetables for vitamin C deficiency, or fatty fish for vitamin D deficiency
  • Nutritional supplements: Oral supplementation with the relevant vitamin at therapeutic doses
  • Intravenous or intramuscular administration: In cases of severe deficiency or absorption disorders, parenteral administration may be necessary (e.g., vitamin B12 injections)
  • Treatment of the underlying condition: If an illness is the root cause, it must be addressed alongside vitamin replenishment

Early recognition and treatment are essential to prevent permanent damage, particularly to the nervous system and bones.

At-Risk Groups

Certain population groups are at higher risk of developing hypovitaminosis:

  • Older adults (reduced absorption, limited sun exposure)
  • Pregnant and breastfeeding women (increased demand)
  • Vegans and vegetarians (especially vitamin B12, vitamin D, and iron)
  • Individuals with chronic gastrointestinal diseases
  • Persons with alcohol dependency
  • Infants (especially vitamin K and D)
