What is Inflammatory Bowel Disease (IBD)?

IBD stands for Inflammatory Bowel Disease, a group of chronic conditions characterized by long-term inflammation of the gastrointestinal tract. The two main forms are Crohn's disease and ulcerative colitis. IBD is a lifelong condition that cannot currently be cured, but it can be managed effectively with modern treatments.

Types of IBD

Crohn's Disease

Crohn's disease can affect any part of the gastrointestinal tract, from the mouth to the anus. It most commonly involves the lower small intestine and the large intestine. The inflammation in Crohn's disease penetrates all layers of the bowel wall and can lead to scarring, fistulas, and strictures.

Ulcerative Colitis

Ulcerative colitis affects only the large intestine (colon) and the rectum. The inflammation is limited to the innermost lining of the bowel wall (mucosa) and frequently causes bloody diarrhea.

Causes

The exact cause of IBD is not yet fully understood. Researchers believe that several factors interact to trigger the disease:

  • Genetic predisposition: IBD tends to run in families, and certain gene variants increase the risk of developing the disease.
  • Immune system dysfunction: In IBD, the immune system reacts excessively to gut bacteria and mistakenly attacks the body's own intestinal tissue.
  • Gut microbiome: Changes in the composition of intestinal bacteria play an important role in the development of IBD.
  • Environmental factors: Western diet, stress, smoking, and certain medications such as antibiotics can influence the risk of IBD.

Symptoms

Symptoms of IBD can vary widely depending on the type and severity of the disease. Common symptoms include:

  • Persistent or recurring diarrhea, sometimes with blood or mucus
  • Abdominal pain and cramping
  • Weight loss and fatigue
  • Fever, particularly during active flares
  • Loss of appetite

IBD can also cause extraintestinal manifestations – symptoms outside the digestive tract affecting the joints, skin, or eyes.

Diagnosis

Diagnosing IBD involves a combination of examinations:

  • Colonoscopy: The most important diagnostic tool, during which tissue samples (biopsies) are taken and examined under a microscope.
  • Blood tests: Inflammatory markers such as CRP and a full blood count provide information about active inflammation.
  • Stool tests: The inflammatory marker calprotectin in the stool is a key indicator of intestinal inflammation.
  • Imaging: MRI or abdominal ultrasound complement the diagnostic workup, particularly for Crohn's disease.

Treatment

While IBD cannot currently be cured, treatment aims to relieve flares, achieve remission (symptom-free periods), and prevent complications.

Medical Treatment

  • Aminosalicylates (e.g. mesalazine): Used primarily for ulcerative colitis.
  • Corticosteroids (e.g. prednisolone): For short-term management of acute flares.
  • Immunosuppressants (e.g. azathioprine, methotrexate): For long-term therapy to suppress the overactive immune response.
  • Biologics (e.g. TNF-alpha inhibitors, integrin inhibitors): Modern antibody-based therapies for moderate to severe cases.

Surgical Treatment

In certain cases – such as severe complications or when medications are no longer effective – surgery may be necessary. In ulcerative colitis, removal of the colon can lead to permanent freedom from symptoms.

Diet and Lifestyle

A balanced diet can improve the well-being of IBD patients. During an active flare, easily digestible foods may be helpful. Smoking significantly worsens the course of Crohn's disease and should be avoided.
