What is an Ileum Resection?

Ileum resection is a surgical procedure in which a segment of the ileum – the final and longest portion of the small intestine – is surgically removed. The ileum is approximately 2–4 meters long and plays a critical role in the absorption of key nutrients, particularly vitamin B12 and bile acids. Depending on the length of bowel removed, the procedure can have significant effects on digestion and overall metabolism.

Indications – When is an Ileum Resection Performed?

An ileum resection may be necessary in a variety of conditions, including:

  • Crohn's disease: A chronic inflammatory bowel disease that frequently affects the terminal ileum, causing strictures, fistulas, or abscesses.
  • Intestinal tumors: Both benign and malignant growths in the ileum.
  • Bowel obstruction (ileus): Mechanical or functional blockage of the intestine.
  • Intestinal perforation: A rupture of the bowel wall due to trauma or inflammation.
  • Small bowel ischemia: Reduced blood supply leading to necrosis of bowel tissue.
  • Meckel's diverticulum: A congenital pouch in the bowel wall that can lead to complications.

Surgical Techniques

The procedure can be performed using two main approaches:

Open Surgery (Laparotomy)

In open surgery, the abdomen is accessed through a larger incision. This approach is preferred in complex or emergency situations.

Minimally Invasive Surgery (Laparoscopy)

The laparoscopic approach uses small incisions with a camera and specialized instruments. It is less invasive, associated with faster recovery, and leaves smaller scars.

After the diseased segment is removed, the remaining ends of the bowel are joined together in a procedure called an anastomosis. In some cases, a temporary or permanent stoma (an artificial bowel opening through the abdominal wall) may be required.

Consequences and Possible Complications

The consequences of ileum resection depend largely on how much bowel is removed:

  • Vitamin B12 deficiency: Since vitamin B12 is absorbed exclusively in the terminal ileum, lifelong supplementation is required when this area is resected.
  • Bile acid malabsorption: Impaired reabsorption of bile acids causes chronic diarrhea (bile acid diarrhea).
  • Short bowel syndrome: Removal of large segments (more than 100 cm) can lead to permanent malabsorption.
  • Kidney stones (oxalate stones): Altered fat digestion leads to increased oxalate absorption.
  • Diarrhea and weight loss
  • Anastomotic leakage: A surgical complication where the bowel connection fails to seal properly.

Follow-Up Care and Rehabilitation

After ileum resection, close medical monitoring is essential and includes:

  • Regular blood tests to check nutrient levels (especially vitamin B12, folate, iron, and fat-soluble vitamins)
  • Individualized dietary therapy provided by a registered dietitian
  • Parenteral nutrition (intravenous nutrient delivery) in cases of severe short bowel syndrome
  • Medication to manage symptoms (e.g., antidiarrheals, bile acid sequestrants)
